Mechanism of Action

Cephradine is a bactericidal first-generation cephalosporin. It inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), preventing peptidoglycan cross-linking and causing cell lysis. The parenteral form achieves higher and more reliable plasma concentrations than oral formulations, making it suitable for moderate-to-severe infections.

Indications

  • Bone and joint infections (osteomyelitis — initial parenteral therapy)
  • Perioperative surgical prophylaxis
  • Step-up therapy when oral cephradine absorption is inadequate

IM reconstitution: Add 4ml sterile water for injection to 500mg vial (yields ~5ml, 100mg/ml). IV: Dilute in 50–100ml NS or 5% dextrose and infuse over 30–60 minutes.

Important Notice for Diabetic Patients

  • Diabetic foot infections (severe): Parenteral cephradine covers MSSA and Streptococcus but lacks MRSA and gram-negative (Pseudomonas, ESBL) coverage — combination therapy or carbapenem may be required for polymicrobial diabetic foot infections
  • IV dextrose diluent: If diluted in 5% dextrose (D5W), account for the glucose load (~5g/100ml) in blood glucose monitoring for insulin-dependent diabetics
  • Renal dose adjustment (diabetic nephropathy):
    CrCl >20 ml/min: full dose every 6 hours
    CrCl 5–20 ml/min: 50% dose reduction or extend interval to every 12 hours
    CrCl <5 ml/min / dialysis: 250mg every 12–24 hours; supplement post-dialysis
  • Nephrotoxicity monitoring: Monitor serum creatinine and electrolytes at baseline and every 48–72 hours during prolonged parenteral therapy in patients with pre-existing diabetic nephropathy
  • Stress hyperglycaemia: Systemic infection requiring parenteral antibiotics frequently causes significant hyperglycaemia — sliding-scale insulin or insulin infusion protocol may be required

Contraindications

  • Hypersensitivity to cephradine or any cephalosporin
  • History of anaphylaxis to penicillins (cross-reactivity possible)
  • Severe renal failure without dose adjustment

Storage
