Mechanism of Action
Cephradine is a bactericidal first-generation cephalosporin. It inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), preventing peptidoglycan cross-linking and causing osmotic cell lysis. Parenteral administration achieves higher and more reliable plasma concentrations than oral formulations, making it suitable for moderate-to-severe infections.

Dosage and Administration

IM: Add 4ml sterile water for injection to 500mg vial (100mg/ml). IV: Dilute in 50–100ml NS or D5W; infuse over 30–60 minutes.
Important Notice for Diabetic Patients
- Diabetic foot infections: Covers MSSA and Streptococcus but not MRSA or gram-negative Pseudomonas/ESBL organisms — combination therapy or broader agent may be needed for polymicrobial diabetic foot infections
- D5W diluent: If diluted in 5% dextrose, account for glucose load (~5g/100ml) in blood glucose monitoring for insulin-dependent patients
- Renal dose adjustment (diabetic nephropathy):
CrCl >20 ml/min: standard dose; CrCl 5–20 ml/min: halve dose or extend interval; CrCl <5 ml/min: 250mg every 12–24 hours; supplement post-haemodialysis - Renal monitoring: Check creatinine and electrolytes at baseline and every 48–72 hours during prolonged parenteral therapy in patients with diabetic nephropathy
- Stress hyperglycaemia: Systemic infections frequently cause significant hyperglycaemia — insulin dose adjustment or infusion protocol may be required during inpatient treatment
Contraindications
- Hypersensitivity to cephradine or any cephalosporin
- Anaphylactic reaction to penicillins (cross-reactivity possible)
- Severe renal failure without dose adjustment
