Abac 100mg/ml Injection (Cephradine)

Generic Name: Cephradine BP 100mg/ml

Manufacturer: Beximco Pharmaceuticals Ltd.

Drug Class: First-Generation Cephalosporin Antibiotic (Parenteral)

Dosage Form: Powder for Injection (Reconstituted to 100mg/ml solution)

Route: Intramuscular (IM) or Intravenous (IV)

Prescription Status: Prescription Only (Rx)

Mechanism of Action

Cephradine is a bactericidal first-generation cephalosporin that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs). Parenteral administration achieves higher and more reliable plasma concentrations than oral formulations, making it suitable for moderate-to-severe infections requiring hospital management.

Spectrum of Activity

SensitivityOrganisms
SusceptibleS. aureus (MSSA), Streptococcus spp., E. coli, Klebsiella pneumoniae, Proteus mirabilis
ResistantMRSA, Pseudomonas aeruginosa, Enterococcus spp., ESBL-producing organisms

Indications

  • Moderate-to-severe respiratory tract infections (pneumonia)
  • Complicated urinary tract infections and pyelonephritis
  • Skin and soft tissue infections, surgical wound infections
  • Bone and joint infections (initial IV therapy)
  • Perioperative surgical prophylaxis

Dosage and Administration

IndicationAdult DoseFrequency
Moderate infections500mg – 1gEvery 6 hours (IM or IV)
Severe infectionsUp to 8g/dayDivided every 4–6 hours (IV)
Surgical prophylaxis1g30–60 min pre-op; repeat at 6 hours

IM: Add 4ml sterile water to 500mg vial (100mg/ml). IV: Dilute in 50–100ml NS or D5W; infuse over 30–60 minutes.

Important Notice for Diabetic Patients

  • Diabetic foot infections: Covers MSSA and Streptococcus — NOT MRSA or gram-negative Pseudomonas/ESBL; combination therapy may be required for polymicrobial infections
  • D5W diluent: Account for glucose load (~5g/100ml) when diluted in dextrose — monitor blood glucose closely in insulin-dependent patients
  • Renal dose adjustment (diabetic nephropathy):
    CrCl >20 ml/min: full dose; CrCl 5–20 ml/min: halve dose or extend interval; CrCl <5 ml/min / dialysis: 250mg every 12–24 hours
  • Renal monitoring: Check creatinine every 48–72 hours during prolonged parenteral therapy in patients with pre-existing nephropathy
  • Stress hyperglycaemia: Systemic infection often causes significant glucose elevation — sliding-scale insulin or infusion protocol may be required

Contraindications

  • Hypersensitivity to cephradine or any cephalosporin
  • Anaphylactic reaction to penicillins
  • Severe renal failure without dose adjustment
