Mechanism of Action
Cefaclor is a bactericidal second-generation cephalosporin. It inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), preventing peptidoglycan cross-linking. The 500mg strength achieves higher tissue concentrations suited for moderate-to-severe infections, particularly those caused by beta-lactamase-producing respiratory pathogens and gram-negative organisms resistant to first-generation cephalosporins.

Important Notice for Diabetic Patients

- Diabetic respiratory infections: Good ambulatory choice for community-acquired pneumonia where H. influenzae is likely; review if no improvement at 48–72 hours
- Renal dose adjustment (diabetic nephropathy): Reduce dose by 50% or extend interval when CrCl <30 ml/min — verify current eGFR before prescribing
- False-positive urine glucose: Use glucose oxidase dipstick only — copper reduction tests give false-positive results with cefaclor
- Serum sickness-like reaction: Higher incidence than other oral cephalosporins — discontinue if fever, rash, arthralgia occur together
- Infection-related hyperglycaemia: Intensify blood glucose monitoring throughout treatment
