Mechanism of Action
Acyclovir is selectively phosphorylated by herpes virus thymidine kinase in infected cells to its active triphosphate form, which inhibits viral DNA polymerase and terminates viral DNA replication. In the eye, acyclovir 3% ointment achieves therapeutic antiviral concentrations in the cornea and aqueous humour, sufficient to treat herpes simplex keratitis (HSK) — the most common cause of infectious corneal blindness worldwide.
Relevance to diabetes: Diabetic patients have impaired corneal wound healing and reduced immunity, making herpes simplex keratitis more severe and recurrent. Early treatment with topical acyclovir is essential to prevent corneal scarring and vision loss.

- Herpes simplex keratitis (dendritic and geographic corneal ulcers)

Dosage and Administration

Technique: Wash hands before use. Gently pull down lower eyelid, apply 1cm ribbon of ointment. Close eye gently for 1–2 minutes. Avoid touching the tip to any surface. Temporary blurred vision after application is normal — do not drive immediately after use.
Special Considerations in Diabetes
- Diabetic patients have impaired corneal epithelial healing — monitor closely for persistent epithelial defects
- Do not use topical corticosteroids concurrently without ophthalmologist guidance — steroids can worsen HSK and cause corneal perforation
- Recurrent HSK is more common in diabetics — consider long-term oral prophylaxis with acyclovir or valacyclovir after treatment
- Any change in vision in a diabetic patient warrants urgent ophthalmological evaluation
Contraindications
- Hypersensitivity to acyclovir
- Not for use in the ear, nose, or on skin (ophthalmic use only)
- Do not use with contact lenses in place
