Mechanism of Action
Adaben Duo combines two complementary acne-fighting agents:
- Adapalene 0.1% — a third-generation synthetic retinoid that binds selectively to retinoic acid receptors (RAR-β and RAR-γ) in keratinocytes. It normalises follicular keratinisation, prevents comedone formation, and has anti-inflammatory properties by inhibiting arachidonic acid metabolite production.
- Benzoyl Peroxide 2.5% — generates free oxygen radicals that are bactericidal against Cutibacterium acnes (formerly Propionibacterium acnes). It also has mild keratolytic activity. Unlike topical antibiotics, benzoyl peroxide does not induce bacterial resistance.
The combination simultaneously targets all four pathogenic factors of acne: follicular hyperkeratinisation, sebum excess, C. acnes proliferation, and inflammation — making it more effective than either agent alone.
Relevance to Diabetes
Acne is more prevalent and often more severe in patients with insulin resistance, polycystic ovary syndrome (PCOS), and Type 2 Diabetes — conditions linked by hyperinsulinaemia stimulating IGF-1 and androgen excess. Effective topical combination therapy reduces the risk of skin infections, which are already more common in diabetic patients.
Indications
- Mild to moderate acne vulgaris (comedonal and inflammatory)
- Acne associated with insulin resistance or PCOS
- Acne with mixed comedonal and papulopustular lesions
Dosage and Administration
Dose: Apply a thin layer to affected areas of the face (and/or trunk) once daily in the evening.
How to use:
- Wash and dry face gently before application
- Apply pea-sized amount to affected areas — avoid eyes, lips, nostrils, and mucous membranes
- Wash hands after application
- Use SPF 30+ sunscreen every morning — adapalene increases photosensitivity
Duration: Improvement typically seen at 8–12 weeks. Continue for at least 3 months. Ongoing maintenance therapy may be required.
Warnings and Precautions
- Skin irritation: Dryness, peeling, erythema and burning are common in first 2–4 weeks — reduce frequency to alternate days if severe
- Sun exposure: Use daily sunscreen; avoid prolonged sun exposure and UV lamp/tanning beds
- Pregnancy: Adapalene is category C — avoid in pregnancy; benzoyl peroxide is category C. Use contraception during treatment
- Do not use on broken, eczematous, or sunburned skin
- Avoid concomitant use with other potentially irritating topical agents (alcohol-based products, abrasives, other retinoids)
