ADMIRA-80 Tablet — Gliclazide 80mg
Generic Name: Gliclazide 80mg (Immediate Release)
Manufacturer: Beximco Pharmaceuticals Ltd.
Drug Class: Sulphonylurea — 2nd Generation Oral Antidiabetic
Dosage Form: Tablet (Immediate Release)
Prescription Status: Prescription Only (Rx)
Mechanism of Action
Gliclazide is a second-generation sulphonylurea that lowers blood glucose by stimulating insulin secretion from pancreatic beta cells. It binds to the sulphonylurea receptor-1 (SUR-1) subunit of ATP-sensitive potassium (KATP) channels, causing channel closure, membrane depolarisation, and calcium-dependent insulin exocytosis. Unlike many older sulphonylureas, gliclazide also has extrapancreatic effects including antioxidant activity (free radical scavenging), reduction of platelet aggregation, and improvement in microvascular endothelial function — effects with potential relevance to diabetic vascular complications.
The 80mg immediate-release tablet requires twice-daily dosing; where once-daily dosing is preferred, the Modified Release (MR) formulation (ADMIRA MR) is generally substituted.
Indications
- Type 2 Diabetes Mellitus (T2DM) in adults
- Monotherapy when diet and exercise are insufficient
- Combination therapy with metformin, alpha-glucosidase inhibitors, or insulin
Dosage and Administration
| Stage | Dose | Timing |
|---|---|---|
| Starting dose | 40–80mg once daily | With breakfast |
| Titration | Increase gradually every 2–4 weeks | Based on blood glucose monitoring |
| Usual maintenance | 80–160mg/day in 2 divided doses | With breakfast and dinner |
| Maximum dose | 320mg/day (in 2–3 divided doses) | Doses >160mg must be split |
Always take with meals. Never skip a meal after taking this tablet. The 80mg tablet may be halved if a lower dose is required.
Warning — Hypoglycaemia Risk
As an immediate-release sulphonylurea, ADMIRA-80 carries a higher hypoglycaemia risk than the MR formulation, particularly if meals are skipped or delayed.
Risk factors: Irregular meals, excessive exercise, alcohol, renal/hepatic impairment, drug interactions, elderly patients.
Symptoms: Sweating, trembling, palpitations, confusion, dizziness, blurred vision.
Treatment: 15–20g fast-acting glucose (sugar, juice, glucose tablets). Unconscious patient: IV glucose 50% or glucagon IM — emergency services immediately.
Monitoring
| Parameter | Target | Frequency |
|---|---|---|
| HbA1c | <7.0% (individualise) | Every 3 months (6-monthly if stable) |
| Fasting blood glucose | 4.4–7.2 mmol/L | Daily during titration |
| 2-hour postprandial glucose | <10 mmol/L (<180 mg/dL) | Weekly during initiation |
| Renal function | eGFR; adjust if impaired | Annually |
Renal and Hepatic Considerations
- Mild-moderate renal impairment: Use with caution; start at lowest dose; monitor closely
- Severe renal impairment (eGFR <30): Avoid — risk of severe prolonged hypoglycaemia
- Hepatic impairment: Use with caution; consider switching to insulin
Contraindications
- Type 1 Diabetes Mellitus or diabetic ketoacidosis
- Severe renal or hepatic impairment
- Pregnancy and breastfeeding
- Hypersensitivity to gliclazide, sulphonylureas, or sulphonamides
- Concomitant miconazole use (oral/systemic)
