Mechanism of Action

Gliclazide stimulates insulin secretion from pancreatic beta cells by binding to the sulphonylurea receptor (SUR-1) on ATP-sensitive potassium (KATP) channels, causing depolarisation and calcium-mediated insulin release. The modified release (MR) formulation delivers gliclazide gradually over 24 hours, producing a more physiological, meal-linked insulin secretory profile compared to immediate-release tablets. It also has extrapancreatic effects including improvement of insulin sensitivity, reduction of platelet aggregation, and antioxidant properties particularly relevant to diabetes-related vascular complications.

Warning — Hypoglycaemia Risk

Although the MR formulation reduces hypoglycaemia risk compared to immediate-release gliclazide, it remains a sulphonylurea and can cause hypoglycaemia.

Risk factors: Skipping meals, excessive alcohol, strenuous exercise, renal/hepatic impairment, drug interactions (azoles, fibrates, NSAIDs), elderly patients.

Symptoms: Sweating, trembling, palpitations, dizziness, confusion, hunger.

Treatment: 15–20g fast-acting carbohydrate. If unconscious: IV dextrose or glucagon — emergency care needed.

Renal and Hepatic Considerations
