Overview

Amdocal PLUS 50 Tablet is the higher-strength fixed-dose combination antihypertensive from Beximco Pharmaceuticals Ltd., containing Amlodipine Besilate 5 mg and Atenolol 50 mg. It provides double the atenolol dose of Amdocal PLUS 25, making it appropriate for patients requiring more pronounced heart rate reduction — including those with hypertension plus significant tachycardia, stable angina with inadequate control, or post-MI patients needing cardioprotective beta-blocker doses.

Indications

  • Hypertension + Tachycardia — stronger heart rate control alongside BP reduction
  • Hypertension + Stable Angina — full 50 mg atenolol dose for superior anti-ischaemic effect
  • Post-MI Hypertension — standard cardioprotective atenolol dose plus vasodilation
  • Hypertension Inadequately Controlled on Amdocal PLUS 25 — step-up therapy

Benefits

  • Full standard atenolol dose (50 mg) + amlodipine 5 mg in one daily tablet
  • Stronger heart rate reduction than Amdocal PLUS 25
  • Single once-daily tablet improves adherence
  • Amlodipine counteracts peripheral vasoconstriction from atenolol
  • Manufactured under WHO-GMP standards by Beximco Pharmaceuticals

Pharmacology

Amlodipine (5 mg): Long-acting dihydropyridine CCB. Selectively inhibits L-type calcium channels in vascular smooth muscle, causing arteriolar vasodilation, reducing peripheral resistance and BP. Half-life 35–50 hours.

Atenolol (50 mg): Cardioselective beta-1 blocker at standard therapeutic dose. At 50 mg, produces significant reduction in resting and exercise heart rate, myocardial contractility, cardiac output, and renin release. The 50 mg dose achieves near-complete beta-1 blockade at rest in most adults. Half-life 6–9 hours; renally eliminated.

Dosage & Administration

IndicationDoseNotes
Hypertension / Angina1 tablet once dailySame time each day; with or without food
Renal Impairment (eGFR 15–35)Every 48 hoursAs directed by physician
Severe Renal Impairment (eGFR <15)Every 96 hoursSpecialist supervision required

Important: Never stop abruptly — taper atenolol gradually to avoid rebound angina or hypertensive crisis.

Special Populations

  • Diabetic Patients: Atenolol 50 mg strongly blunts hypoglycaemia tachycardia warning. Frequent glucose monitoring essential.
  • Elderly: Increased risk of bradycardia and falls. Start with Amdocal PLUS 25.
  • Renal Impairment: Atenolol accumulates in CKD — extend dosing interval. Amlodipine needs no renal adjustment.
  • Pregnancy: Avoid — atenolol is Category D (fetal growth restriction).

Contraindications

  • Bradycardia (HR <50 bpm), sick sinus syndrome, 2nd/3rd degree AV block
  • Cardiogenic shock or decompensated heart failure
  • Severe hypotension (systolic <90 mmHg)
  • Significant asthma/COPD
  • Hypersensitivity to amlodipine or atenolol

Key Drug Interactions

DrugInteraction
Verapamil / DiltiazemSevere bradycardia and AV block risk. Avoid.
Insulin / SulfonylureasAtenolol masks hypoglycaemia tachycardia — monitor glucose closely.
ClonidineRebound hypertension on clonidine withdrawal — taper clonidine first.
NSAIDsReduce antihypertensive effect of both components.
CYP3A4 InhibitorsMay increase amlodipine exposure; monitor for hypotension.

Side Effects

Common: Fatigue, bradycardia, cold extremities, peripheral oedema, dizziness, flushing.

Uncommon: Nausea, nightmares, depression, bronchospasm (susceptible patients), insomnia.

Rare: AV block, severe hypotension, Raynaud's phenomenon, psoriasiform rash.

Storage

  • Store below 30°C, protect from moisture and sunlight
  • Keep in original blister pack; keep out of reach of children

Frequently Asked Questions

Q1: What is Amdocal PLUS 50 used for?
It treats hypertension and angina where stronger heart rate control is needed than Amdocal PLUS 25 provides. Also used in post-MI patients requiring standard cardioprotective atenolol doses (50 mg).

Q2: How is it different from Amdocal PLUS 25?
Both contain amlodipine 5 mg. Amdocal PLUS 50 has atenolol 50 mg vs 25 mg — providing stronger heart rate reduction and beta-blockade, suitable for faster resting HRs or more severe angina.

Q3: Can diabetic patients take Amdocal PLUS 50?
With caution. Atenolol 50 mg significantly blunts the tachycardia warning of hypoglycaemia. Patients on insulin or sulfonylureas must monitor glucose more frequently and discuss with their physician.

Q4: Can I stop it abruptly?
Never. Sudden withdrawal of atenolol can trigger rebound angina, MI, or hypertensive crisis. Taper gradually under physician guidance over 1–2 weeks.

Q5: Safe for asthma/COPD patients?
No. Even at 50 mg, atenolol may trigger bronchospasm in significant airway disease. Discuss safer alternatives with your physician.

Q6: Is it prescription-only?
Yes — Rx medicine. Upload your prescription when ordering from DiabetesStore.com.bd.
