What is Bisopro Plus?
Bisopro Plus combines Bisoprolol Fumarate, a selective beta-1 blocker, with Hydrochlorothiazide, a thiazide diuretic, for the treatment of hypertension, manufactured by Incepta Pharmaceuticals Ltd.
Indications
Indicated in the treatment of hypertension.
Pharmacology
Bisoprolol and Hydrochlorothiazide have additive antihypertensive effects. Hydrochlorothiazide 6.25 mg significantly increases the antihypertensive effect of Bisoprolol with a lower incidence of hypokalemia than higher-dose Hydrochlorothiazide. Bisoprolol is a beta-1 selective adrenoceptor blocker; Hydrochlorothiazide increases excretion of sodium and chloride via renal tubular mechanisms.
Dosage & Administration
Initial therapy: one 2.5/6.25 mg tablet once daily, titrated at 14-day intervals up to a maximum of 20/12.5 mg once daily.
Replacement therapy: may substitute for titrated individual components.
Interaction
May potentiate other antihypertensives. Should not be combined with other beta-blockers. Caution with catecholamine-depleting drugs, clonidine, calcium antagonists (verapamil, diltiazem), antiarrhythmics (disopyramide), and digitalis glycosides.
Contraindications
Cardiogenic shock, overt cardiac failure, second/third-degree AV block, marked sinus bradycardia, anuria, hypersensitivity to either component or other sulfonamide-derived drugs.
Side Effects
Fatigue, dizziness, headache, bradycardia, arrhythmia, peripheral ischemia, chest pain, palpitations, cold extremities, orthostatic hypotension, diarrhea, constipation, nausea, dyspepsia, rhinitis, pharyngitis.
Pregnancy & Lactation
Pregnancy Category C; use only if benefit justifies risk. Thiazides are excreted in breast milk; decision needed whether to discontinue nursing or the drug.
Precautions & Warnings
Risk of hyperuricemia/gout with thiazides. Watch for fluid/electrolyte imbalance signs. Withdraw gradually over about 2 weeks if discontinuing.
