What is Budemet?

Budemet is a combination dry powder inhaler containing Budesonide (an inhaled corticosteroid) and Formoterol Fumarate Dihydrate (a long-acting beta2-agonist), used for the maintenance treatment of asthma and COPD. It is intended for oral inhalation via dry powder inhaler only and must not be swallowed.

Indications

Asthma: Maintenance treatment of asthma in patients 12 years of age and older (not indicated for relief of acute bronchospasm).

COPD: Twice-daily maintenance treatment of airflow obstruction in patients with chronic obstructive pulmonary disease (COPD), including chronic bronchitis and emphysema.

Pharmacology

Budesonide is a glucocorticosteroid with potent anti-inflammatory activity, inhibiting multiple cell types and mediators involved in allergic and non-allergic airway inflammation.

Formoterol Fumarate Dihydrate is a selective long-acting beta2-adrenoceptor agonist that relaxes bronchial smooth muscle and inhibits hypersensitivity mediator release. Onset of bronchodilation is within 1-3 minutes and duration is at least 12 hours after a single dose.

Dosage & Administration

Asthma — Adults and adolescents 12 years and older: Maintenance: 1 capsule twice daily. Reliever: 1 additional capsule as needed (max 6 per occasion; total daily dose up to 12 capsules temporarily if needed).

COPD — Adults 40 years and older: 2 capsules (200 mcg strength) twice daily, maximum 4 capsules/day. Use with appropriate dry powder inhaler device only; capsules must not be swallowed.

Interaction

Strong CYP3A4 inhibitors (e.g. ritonavir, ketoconazole, itraconazole) should be avoided due to increased systemic corticosteroid effects. Extreme caution with MAO inhibitors and tricyclic antidepressants. Beta-blockers may block bronchodilatory effects and cause severe bronchospasm. Non-potassium-sparing diuretics may worsen hypokalemia.

Contraindications

Not for primary treatment of status asthmaticus or acute asthma/COPD episodes requiring intensive measures. Contraindicated in patients with hypersensitivity to any ingredient.

Side Effects

Formoterol-related: Tremor, palpitations, headache (common); arrhythmias, muscle cramps, hypersensitivity reactions (uncommon/rare). Budesonide-related: Hoarseness, oropharyngeal candidiasis, throat irritation; growth effects in children, glaucoma, cataract (rare).

Pregnancy & Lactation

Budesonide is excreted in breast milk, but at therapeutic doses no effects on the suckling child are anticipated. Administer only when expected benefit to the mother outweighs any possible risk to the child.

Precautions & Warnings

Use with caution in diabetes mellitus, thyrotoxicosis, untreated hypokalaemia, severe hypertension, ischaemic heart disease, tachycardia, severe heart failure, glaucoma, and cataract. Discontinue immediately if paradoxical bronchospasm occurs. Monitor height in children receiving prolonged corticosteroid treatment.

Overdose Effects

Formoterol overdose: tremor, headache, palpitations, tachycardia, arrhythmia — supportive and symptomatic treatment. Acute Budesonide overdose is not clinically significant; chronic excess may cause systemic glucocorticoid effects.

Storage Conditions

Store at or below 25°C, away from light and moisture. Remove capsule from blister only immediately before use. Keep out of reach of children.
