Overview
Isovent (Misoprostol) 200 mcg Tablet is a synthetic prostaglandin E1 analogue manufactured by Square Pharmaceuticals PLC. It is indicated for the prophylaxis and healing of NSAID-induced gastric and duodenal ulcers, healing of benign gastric and duodenal ulcers, induction of labor, and prevention and treatment of postpartum hemorrhage. Dosage and Administration
For benign gastric/duodenal ulceration and NSAID-associated ulceration: 800 mcg daily in 2–4 divided doses with meals and at bedtime for at least 4 weeks (up to 8 weeks if needed). For prophylaxis of NSAID-induced ulcer: 200 mcg 2–4 times daily taken with NSAID therapy. For induction of labor: 25 mcg vaginally every 6 hours (max 200 mcg total) or 100 mcg orally repeated every 4 hours up to 6 doses. For prevention of postpartum hemorrhage: 600 mcg orally immediately after delivery. For treatment of postpartum hemorrhage: 600 mcg orally or 1000 mcg rectally. Interaction
There is no evidence of clinically significant interaction between Isovent and cardiac, pulmonary, CNS drugs, or NSAIDs. High doses of antacids may decrease the bioavailability of Misoprostol.
Contraindications
Isovent is contraindicated in patients with a history of allergy to prostaglandins. It is also contraindicated during pregnancy when used for NSAID-associated ulcer prophylaxis due to its abortifacient properties. Diarrhea may be minimized by taking the drug after meals and at bedtime, and by avoiding concomitant magnesium-containing antacids.
Pregnancy and Lactation
Misoprostol is contraindicated in pregnant women due to its abortifacient properties. It should only be prescribed to women of childbearing potential who require NSAID therapy and are at high risk of ulcer complications, provided they have a negative pregnancy test, use effective contraception, and fully understand the risks. Excretion into breast milk is possible and Isovent is not recommended for nursing mothers.
