What is A-Mectin 6mg (Ivermectin)?

A-Mectin 6 mg contains Ivermectin, a broad-spectrum antiparasitic drug and WHO Essential Medicine. The 6 mg strength allows flexible weight-based dosing — particularly useful for patients requiring 6 mg (body weight ~30 kg) or as part of combination dosing for intermediate-weight patients. Ivermectin has been used globally since the 1980s and remains the drug of choice for scabies, strongyloidiasis, onchocerciasis, and head lice. A-Mectin is manufactured by ACI Limited, Bangladesh.

Mechanism of Action

Ivermectin selectively binds to glutamate-gated chloride ion channels unique to invertebrate nerve and muscle cells. This causes hyperpolarisation, flaccid paralysis, and death of the parasite. These channels are absent in mammals, giving ivermectin an excellent safety profile in humans. It also enhances GABA-mediated neurotransmission in parasites, further impairing their neurological function.

Indications

  • Scabies (Sarcoptes scabiei): Oral treatment, especially for crusted scabies or topical treatment failure. Two doses 7–14 days apart recommended
  • Head lice (Pediculosis capitis): Effective oral alternative when permethrin or malathion fail
  • Strongyloidiasis: Intestinal roundworm — single dose, >95% cure rate
  • Onchocerciasis: River blindness — first-line treatment
  • Lymphatic filariasis: Mass drug administration programs
  • Cutaneous larva migrans: Creeping eruption from animal hookworm larvae

Dosage — Weight-Based Calculation

Standard dose: 200 mcg/kg body weight as a single oral dose.

Dosing guide for A-Mectin 6 mg:

  • Body weight 15–24 kg → 3 mg (half tablet)
  • Body weight 25–35 kg → 6 mg (1 tablet)
  • Body weight 36–50 kg → 9 mg (1½ tablets)
  • Body weight 51–65 kg → 12 mg (2 tablets or switch to A-Mectin 12 mg)

Scabies: Two doses 7–14 days apart. Single dose alone often insufficient as eggs are unaffected.

Administration: Take on an empty stomach with water, at least 2 hours before or after food. Fatty meals increase absorption 2.5× — take fasting for predictable dosing.

Contraindications

  • Hypersensitivity to ivermectin
  • Children under 15 kg body weight
  • Pregnancy (Category C) — avoid unless clearly necessary
  • Loiasis — assess for Loa loa infection in patients from endemic areas

Side Effects

Mazzotti reaction (onchocerciasis patients): fever, rash, itching, swelling — a die-off reaction from killing microfilariae, not true toxicity.

GI: Nausea, abdominal discomfort, diarrhoea — mild and transient.

CNS (rare): Dizziness, somnolence — mainly with very high doses or MDR1 gene variants affecting blood-brain barrier.

Very rare: Serious neurological events in high Loa loa microfilaraemia.

Drug Interactions

  • Warfarin: May enhance anticoagulant effect — monitor INR closely
  • P-gp inhibitors (ketoconazole): May increase ivermectin levels

Frequently Asked Questions (FAQ)

Q: Why do I need two doses of ivermectin for scabies?
A: The first dose kills active mites. However, ivermectin does not kill mite eggs (it is not ovicidal). The second dose 7–14 days later targets the new generation of mites that hatch from those eggs, ensuring complete eradication.

Q: How many A-Mectin 6mg tablets should I take?
A: It depends on your weight. The dose is 200 mcg/kg. For a 30 kg person: 6 mg (1 tablet). For a 60 kg person: 12 mg (2 tablets). For a 90 kg person: ~18 mg (3 tablets). Always follow your doctor's prescribed dose.

Q: Can A-Mectin 6mg be used for children?
A: Yes, for children weighing 15 kg or more. The 6 mg strength is particularly useful for paediatric weight-based dosing. Do not use in children under 15 kg.

Q: Is it safe to take ivermectin if I am on blood thinners (warfarin)?
A: Use with caution. Ivermectin can enhance warfarin's anticoagulant effect. Inform your doctor if you are on warfarin — your INR should be monitored after taking ivermectin.
