Prescription StatusPrescription Required

Overview

Abetis 40mg is the maximum approved dose of Olmesartan Medoxomil — a potent, selective AT1 Angiotensin II receptor blocker (ARB). The 40mg dose is used when lower doses (10–20mg) fail to achieve target blood pressure. It is particularly important in diabetic patients with resistant hypertension or significant proteinuria, where maximum RAS blockade provides the greatest renoprotective benefit.

Mechanism of Action

Olmesartan is hydrolysed after oral absorption to its active form (RNH-6270), which selectively and insurmountably blocks the AT1 receptor. This prevents angiotensin II from causing vasoconstriction, aldosterone secretion, sodium retention, and renal afferent arteriolar constriction. At 40mg, the AT1 receptor is blocked more completely and for a longer duration, providing superior blood pressure control throughout the 24-hour dosing interval and greater reduction in intraglomerular hypertension — the key driver of diabetic nephropathy progression.

Indications

  • Resistant hypertension — when 20mg dose is insufficient
  • Diabetic nephropathy with significant proteinuria — maximum renoprotection
  • High cardiovascular risk patients requiring tight blood pressure control
  • Combination therapy (with amlodipine or HCTZ) in more severe hypertension

Dosage

IndicationDoseNotes
Resistant hypertension40mg once dailyMaximum dose — do not exceed
Diabetic nephropathy40mg once dailyMost effective dose for proteinuria reduction
UptitrationStart 20mg → increase to 40mg after 2 weeks if neededFull effect seen at 2 weeks per dose

Take with or without food at the same time each day. 40mg is the ceiling dose — exceeding it adds no further benefit.

Critical Diabetic Patient Information

  • Maximum renoprotection at 40mg: Studies show 40mg olmesartan reduces urinary albumin excretion (proteinuria) more effectively than lower doses. Diabetic patients with macroalbuminuria or declining eGFR benefit most from the 40mg dose
  • Heightened hyperkalaemia risk: The 40mg dose suppresses aldosterone more completely — potassium monitoring is especially important. Check K⁺ within 1 week of dose increase to 40mg and every 3 months thereafter, particularly if eGFR <60
  • Renal function monitoring: Check creatinine 1–2 weeks after dose increase. A rise ≤30% is acceptable. Greater rises warrant specialist review
  • Blood pressure at target: At 40mg, many patients reach <130/80 mmHg. Hypotension risk increases — monitor especially in the elderly, volume-depleted, or those on diuretics
  • Absolute prohibition of dual RAS blockade: Never combine with ACE inhibitor or aliskiren, especially at maximum ARB dose

Side Effects

SystemSide EffectSeverity
RenalElevated creatinine (expected — stabilises)Common — usually acceptable
ElectrolytesHyperkalaemia (K⁺ >5.5 mEq/L) — more likely at 40mgModerate — monitor closely
CardiovascularHypotension, dizziness, orthostatic symptomsCommon — Mild to Moderate
GIOlmesartan-associated enteropathy (severe diarrhoea — rare)Rare — may require discontinuation
Serious (rare)AngioedemaRare — discontinue immediately

Drug Interactions

DrugInteraction
ACE inhibitorsContraindicated combination — dual RAS blockade
AliskirenContraindicated in diabetics (ALTITUDE trial)
Potassium-sparing diureticsSignificant hyperkalaemia risk at 40mg dose
NSAIDsBlunt antihypertensive effect; worsen nephrotoxicity
Insulin / sulfonylureasMonitor glucose — improved insulin sensitivity may require dose adjustment
LithiumIncreased lithium toxicity

Contraindications

  • Pregnancy (second and third trimester)
  • Known hypersensitivity to olmesartan
  • Combination with aliskiren in diabetics or eGFR <60
  • Bilateral renal artery stenosis
  • Severe hepatic impairment

Frequently Asked Questions

Q: Is 40mg olmesartan safe?
A: Yes, when prescribed and monitored appropriately. It is the maximum approved dose and is very effective, but requires closer monitoring of renal function and potassium than lower doses.

Q: My doctor increased my dose to 40mg — what should I watch for?
A: Dizziness (especially on standing), which usually resolves within 1–2 weeks. Get a blood test (renal function + potassium) within 1–2 weeks of the dose increase. Report any severe diarrhoea immediately.

Q: Can I take Abetis 40mg with my diabetes medicines?
A: Yes, but ARBs may improve insulin sensitivity — monitor blood glucose more closely after starting, as your antidiabetic dose may need adjustment.

Medical Disclaimer: Abetis 40mg (Olmesartan Medoxomil) is a prescription-only medicine. Contraindicated in pregnancy. Requires regular monitoring of renal function and potassium.
