Overview

Acuren 50mg is the higher-strength formulation of Hydrochlorothiazide (HCTZ), a thiazide diuretic used for hypertension and oedema management. The 50mg dose is used where 25mg provides insufficient response, particularly in oedema states. However, higher HCTZ doses carry greater risk of metabolic side effects — electrolyte disturbances, glucose elevation, and dyslipidaemia are more pronounced at 50mg. Diabetic patients require heightened vigilance with this dose.

Mechanism of Action

HCTZ inhibits the Na⁺/Cl⁻ cotransporter in the distal convoluted tubule, reducing sodium and water reabsorption. This increases urinary output and reduces plasma volume, lowering blood pressure. Critical Diabetic Patient Warnings

  • Greater hyperglycaemia risk at 50mg: The metabolic effects of HCTZ are dose-dependent — the 50mg dose causes more significant blood glucose elevation than 25mg. Fasting glucose and HbA1c must be monitored closely and antidiabetic doses adjusted as needed
