Role of Calcium and Vitamin D3

Calcium is the primary mineral in bone. Vitamin D3 (Cholecalciferol) is converted to calcitriol, which enhances intestinal calcium absorption and regulates PTH, ensuring adequate bone mineralisation.

Critical Relevance to Diabetes

  • Vitamin D deficiency is highly prevalent in Type 2 Diabetes and linked to insulin resistance
  • Diabetic patients have higher fracture risk due to calcium wasting via glucosuria and fall risk from neuropathy
  • Long-term metformin may impair calcium and vitamin B12 absorption
  • Pioglitazone and insulin therapy associated with reduced bone mineral density
  • Vitamin D supplementation may improve glycaemic control in vitamin D-deficient diabetic patients

Indications

  • Calcium and vitamin D3 deficiency prevention and treatment
  • Osteoporosis prevention and adjunct treatment
  • Elderly patients with limited sun exposure
  • Diabetic patients on long-term metformin, thiazolidinediones, or corticosteroids
  • Pregnancy and breastfeeding supplementation

Dosage

1–2 tablets daily with meals. Space at least 2 hours from bisphosphonates, levothyroxine, and iron supplements.

Contraindications

  • Hypercalcaemia or hypercalciuria
  • Calcium kidney stones (nephrolithiasis)
  • Severe renal impairment (eGFR <30)
  • Hypervitaminosis D

Drug Interactions

  • Bisphosphonates, levothyroxine, iron — take 2–4 hours apart
  • Thiazide diuretics — hypercalcaemia risk
  • Digoxin — hypercalcaemia potentiates toxicity

Side Effects

  • Constipation, bloating (common with calcium carbonate)
  • Hypercalcaemia with overdose: thirst, polyuria, confusion
