Sildenafil Citrate is a selective inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type-5 (PDE5). By inhibiting PDE5, sildenafil increases cGMP levels in the corpus cavernosum, relaxing smooth muscle and allowing increased blood flow necessary for an erection. It is also used to improve exercise ability in patients with pulmonary arterial hypertension.
Indications
Adegra (Sildenafil Citrate) is indicated for:
- Erectile Dysfunction (ED): Treatment of male sexual function problems (impotence or erectile dysfunction) in adult men.
- Pulmonary Arterial Hypertension (PAH): To improve exercise ability and delay clinical worsening in patients with WHO Group I PAH.
Dosage & Administration
Erectile Dysfunction: 50mg taken as needed approximately 1 hour before sexual activity. Based on effectiveness and tolerability, the dose may be increased to a maximum of 100mg or decreased to 25mg. The maximum recommended dosing frequency is once per day. Adegra 25mg is the lower dose formulation suitable for patients who require a reduced starting dose.
Pulmonary Hypertension: 20mg three times daily. (Typically higher-dose formulations are used for this indication — consult prescriber.)
Administration: May be taken with or without food. Avoid high-fat meals immediately before taking as they can delay absorption.
Contraindications & Precautions
Adegra is contraindicated in patients:
- Taking nitrates in any form (e.g., nitroglycerin, isosorbide mononitrate/dinitrate) — can cause severe hypotension
- With hypersensitivity to sildenafil or any excipient
- With severe hepatic impairment
- With recent stroke or myocardial infarction, or with low blood pressure (<90/50 mmHg)
Use with caution when co-administered with alpha-blockers (additive blood pressure lowering effect). Patients with anatomical deformation of the penis (angulation, cavernosal fibrosis, Peyronie's disease) or conditions predisposing to priapism (sickle cell anaemia, multiple myeloma, leukaemia) should use with caution.
Side Effects
Common side effects include headache, flushing of the face, and upset stomach. Less common effects may include temporary changes in color vision, increased sensitivity to light, or blurred vision. Seek immediate medical attention if you experience a sudden decrease or loss of vision or hearing, chest pain, or an erection lasting more than 4 hours (priapism).
Drug Interactions
Significant interactions include nitrates (contraindicated), alpha-blockers (use with caution, may cause hypotension), CYP3A4 inhibitors (e.g., ritonavir, ketoconazole — reduce sildenafil dose), and other PDE5 inhibitors. Avoid grapefruit juice.
Storage
