Sildenafil Citrate is a selective inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type-5 (PDE5). By inhibiting PDE5, sildenafil increases cGMP levels in the corpus cavernosum, relaxing smooth muscle and allowing increased blood flow necessary for an erection. The 50mg tablet is the standard recommended starting dose for most patients.

Indications

Adegra 50mg (Sildenafil Citrate) is indicated for:

  • Erectile Dysfunction (ED): Treatment of male sexual function problems (impotence or erectile dysfunction) in adult men.
  • Pulmonary Arterial Hypertension (PAH): To improve exercise ability in patients with WHO Group I PAH (20mg dose formulation typically used — consult prescriber).

Dosage & Administration

Erectile Dysfunction: The recommended dose is 50mg taken as needed approximately 1 hour before sexual activity. Based on effectiveness and tolerability, the dose may be increased to a maximum of 100mg or decreased to 25mg. The maximum recommended dosing frequency is once per day.

Administration: May be taken with or without food. Avoid high-fat meals immediately before taking as they may delay absorption and reduce effectiveness.

Contraindications & Precautions

Adegra is contraindicated in patients:

  • Taking nitrates in any form (e.g., nitroglycerin, isosorbide mononitrate/dinitrate) — can cause severe, potentially fatal hypotension
  • With hypersensitivity to sildenafil or any excipient
  • With severe hepatic impairment
  • With recent stroke or myocardial infarction, or with resting hypotension (BP <90/50 mmHg)

Use with caution when co-administered with alpha-blockers — additive blood pressure lowering effect may cause symptomatic hypotension. Use with caution in patients with anatomical deformation of the penis or conditions predisposing to priapism (sickle cell anaemia, multiple myeloma, leukaemia).

Side Effects

Common side effects include headache, flushing of the face, and dyspepsia (upset stomach). Less common effects may include temporary changes in color vision, increased sensitivity to light, or blurred vision. Seek immediate medical attention for sudden vision/hearing loss, chest pain, or prolonged erection (>4 hours).

Drug Interactions

Contraindicated with nitrates. Use with caution with alpha-blockers, CYP3A4 inhibitors (ritonavir, ketoconazole — dose reduction required), and other antihypertensives. Avoid concurrent use with other PDE5 inhibitors. Grapefruit juice may increase sildenafil levels.

Storage

Store below 30°C, away from moisture and direct sunlight. Keep out of reach of children. Do not use after the expiry date printed on the pack.
