Cefixime is a broad-spectrum, third-generation oral cephalosporin antibiotic. It exerts its bactericidal effect by inhibiting bacterial cell wall synthesis through binding to penicillin-binding proteins (PBPs), leading to cell lysis and death. It is stable against many beta-lactamases and active against a wide range of gram-negative organisms.

Indications

Adexim 200 (Cefixime Trihydrate) is indicated for treatment of infections caused by susceptible organisms:

  • Urinary Tract Infections (UTI): Uncomplicated UTIs caused by E. coli and Proteus mirabilis
  • Respiratory Tract Infections: Acute bronchitis, acute exacerbations of chronic bronchitis
  • Otitis Media: Caused by Haemophilus influenzae, Moraxella catarrhalis, or Streptococcus pyogenes
  • Pharyngitis & Tonsillitis caused by Streptococcus pyogenes
  • Typhoid Fever: Uncomplicated typhoid in susceptible cases
  • Gonorrhoea: Uncomplicated cervical/urethral gonorrhoea

Dosage & Administration

Adults: 200–400mg daily, either as a single dose or divided into two equal doses. For most infections, 200mg twice daily or 400mg once daily.

Children: 8mg/kg/day in a single or two divided doses. (Use suspension formulation for children.)

Duration: As prescribed by the physician. Do not stop the course early even if symptoms improve.

Administration: May be taken with or without food. Food may reduce GI side effects.

Contraindications & Precautions

  • Contraindicated in patients with known hypersensitivity to cefixime, other cephalosporins, or penicillins (cross-sensitivity possible)
  • Use with caution in patients with renal impairment — dose adjustment may be required (CrCl <60 mL/min)
  • Use with caution in patients with GI disease, particularly colitis
  • Monitor for superinfection with prolonged use

Side Effects

Common: Diarrhoea, loose stools, nausea, abdominal pain, dyspepsia, and flatulence. Hypersensitivity reactions (rash, urticaria, pruritus) may occur rarely. Serious but rare: Clostridioides difficile-associated diarrhoea, Stevens-Johnson syndrome.

Drug Interactions

Cefixime may increase anticoagulant effect of warfarin. Carbamazepine levels may increase. Probenecid may increase cefixime serum levels.

Storage

Store below 30°C in a cool, dry place away from light and moisture. Keep out of reach of children.
