Mechanism of Action

Gliclazide is a second-generation sulphonylurea that lowers blood glucose by stimulating insulin secretion from pancreatic beta cells. It binds to the sulphonylurea receptor-1 (SUR-1) subunit of ATP-sensitive potassium (KATP) channels, causing channel closure, membrane depolarisation, and calcium-dependent insulin exocytosis. Unlike many older sulphonylureas, gliclazide also has extrapancreatic effects including antioxidant activity (free radical scavenging), reduction of platelet aggregation, and improvement in microvascular endothelial function — effects with potential relevance to diabetic vascular complications.

The 80mg immediate-release tablet requires twice-daily dosing; where once-daily dosing is preferred, the Modified Release (MR) formulation (ADMIRA MR) is generally substituted.

Indications

  • Type 2 Diabetes Mellitus (T2DM) in adults
  • Monotherapy when diet and exercise are insufficient
  • Combination therapy with metformin, alpha-glucosidase inhibitors, or insulin

Always take with meals. Never skip a meal after taking this tablet. The 80mg tablet may be halved if a lower dose is required.

Warning — Hypoglycaemia Risk

As an immediate-release sulphonylurea, ADMIRA-80 carries a higher hypoglycaemia risk than the MR formulation, particularly if meals are skipped or delayed.

Risk factors: Irregular meals, excessive exercise, alcohol, renal/hepatic impairment, drug interactions, elderly patients.

Symptoms: Sweating, trembling, palpitations, confusion, dizziness, blurred vision.

Treatment: 15–20g fast-acting glucose (sugar, juice, glucose tablets). Unconscious patient: IV glucose 50% or glucagon IM — emergency services immediately.

Renal and Hepatic Considerations
