Indications

Aknill 0.05% Gel is indicated for the topical treatment of:

• Acne vulgaris — including comedonal (blackheads, whiteheads), papular, and mild-to-moderate inflammatory acne
• Reduction of follicular plugging and prevention of microcomedone formation

Topical isotretinoin is particularly suited for patients with comedonal or mild inflammatory acne who wish to avoid or cannot tolerate systemic retinoids. It may be used as monotherapy or in combination with topical antibiotics or benzoyl peroxide.

Composition

Each gram of Aknill Gel contains: Isotretinoin 0.5mg (0.05% w/w) in an aqueous gel base.

Mechanism of Action

Isotretinoin is an all-trans isomer of retinoic acid (Vitamin A acid), a first-generation retinoid. Topical isotretinoin acts through several complementary pathways:

• Normalises follicular keratinisation: Reduces the cohesiveness of follicular keratinocytes, preventing the formation of microcomedones (the precursor to all acne lesions). Promotes shedding of the stratum corneum, clearing existing comedones.
• Reduces sebum production: Downregulates sebaceous gland activity and sebum secretion — removes the nutrient substrate for Cutibacterium acnes (formerly Propionibacterium acnes) proliferation.
• Anti-inflammatory: Reduces the chemotaxis of neutrophils and monocytes; decreases toll-like receptor 2 (TLR2) expression on sebocytes, which is involved in the innate immune response to C. acnes.
• Retinoic acid receptor (RAR) binding: Binds RAR-α and RAR-γ in keratinocytes, modulating gene expression related to cell differentiation and proliferation.

Systemic absorption through intact skin with topical isotretinoin is extremely low — significantly safer than oral isotretinoin, without the systemic teratogenic risk (with appropriate precautions).

Dosage and Administration

Apply a thin film of Aknill 0.05% Gel to the affected areas of the face (or other affected skin) once daily, preferably at bedtime, after gently washing and drying the skin.

Technique: Use a fingertip or a cotton swab to apply sparingly — a pea-sized amount is sufficient for the entire face. Avoid the eyes, mouth, nostrils, and mucous membranes.

Duration: Clinical improvement is usually seen after 4–8 weeks; continue for 12–16 weeks as directed by the physician. Premature discontinuation may result in relapse.

Sun exposure: Retinoids increase photosensitivity — always apply a broad-spectrum sunscreen (SPF ≥30) in the morning during treatment. Avoid prolonged sun exposure and UV lamps.

Combination therapy: May be used in combination with topical benzoyl peroxide (applied separately — different times of day to avoid oxidative inactivation) or topical antibiotics. Do not use concurrently with other retinoids (tretinoin, adapalene).

Contraindications

• Known hypersensitivity to isotretinoin, other retinoids, vitamin A, or any component of the gel
• Pregnancy — topical isotretinoin is contraindicated in pregnancy due to potential teratogenicity (though systemic absorption is low, the risk cannot be entirely excluded)
• Application to eczematous, sunburned, or abraded skin
• Perioral/periocular application

Warnings and Precautions

• Pregnancy: Women of childbearing potential must use effective contraception during treatment. While systemic absorption is much lower than with oral isotretinoin, topical retinoids are teratogenic and must not be used during pregnancy. Confirm absence of pregnancy before starting treatment.
• Photosensitivity: Avoid excessive sun exposure, UV lamps, and tanning beds. Use sunscreen daily.
• Skin irritation: Retinoid dermatitis (erythema, dryness, peeling, burning) may occur — particularly in the first 4–8 weeks. Start with alternate-day application if sensitivity is high; use a gentle moisturiser.
• Eye/mucous membrane contact: Avoid; if accidental contact occurs, rinse thoroughly with water.
• Waxing: Avoid waxing hair from areas treated with retinoids — skin may be more prone to tearing and irritation.
• Keratolytics: Avoid concurrent use of peeling agents (glycolic acid, salicylic acid, benzoyl peroxide on the same application) which may increase irritation.

Side Effects

Very common (at application site): Dryness, peeling, erythema, burning/stinging — especially in first 2–4 weeks of treatment; usually subside as skin adapts.

Common: Pruritus (itching), photosensitivity, increased sensitivity to wind and cold.

Uncommon: Contact dermatitis, exacerbation of acne in the initial weeks ("purging" — a retinoid-induced acceleration of comedone clearance).

Rare: Significant systemic absorption leading to systemic retinoid effects is extremely unlikely with correct use on limited skin areas.

Drug Interactions

• Other topical retinoids (tretinoin, adapalene, tazarotene): Do not combine — additive irritation and no additional benefit.
• Benzoyl peroxide: Apply at different times (e.g., retinoid at night, benzoyl peroxide in the morning) — concurrent application inactivates isotretinoin via oxidation.
• Vitamin A supplements: Avoid excessive Vitamin A supplementation during retinoid therapy to prevent additive toxicity risk.
• Tetracyclines (systemic): Theoretical risk of increased intracranial pressure (pseudotumour cerebri) when combined with systemic tetracyclines; not a documented risk with topical isotretinoin at standard doses, but inform prescriber.

Use in Pregnancy and Lactation

Pregnancy: Contraindicated. All retinoids are teratogenic — use highly effective contraception and discontinue immediately if pregnancy occurs or is suspected. Consult a physician immediately.

Lactation: Avoid use during breastfeeding; do not apply to the breast or nipple area. Systemic absorption is low but precautionary avoidance is recommended.

Storage

Store below 25°C away from direct sunlight and heat. Keep tube tightly closed — retinoids degrade on exposure to light and oxygen. Keep out of reach of children. Do not use after the expiry date printed on the tube.
