Overview
Arlin 400mg contains Linezolid 400mg — an oxazolidinone antibiotic active against drug-resistant Gram-positive bacteria including MRSA, VRE, and penicillin-resistant streptococci. The 400mg strength is used for uncomplicated skin and soft tissue infections (SSTIs) in adults (where 400mg BD achieves adequate tissue concentrations for mild-moderate infections), and as the standard dose for paediatric patients dosed by weight. Linezolid 400mg twice daily provides serum concentrations effective against most Gram-positive pathogens while reducing the risk of adverse effects compared to the 600mg dose used for more severe infections. Linezolid's near-complete oral bioavailability (~100%) allows effective outpatient oral treatment of infections that would otherwise require IV therapy.
Mechanism of Action
Linezolid inhibits bacterial protein synthesis by uniquely binding to the 23S ribosomal RNA of the 50S subunit, preventing formation of the 70S initiation complex — a mechanism entirely distinct from all other antibiotic classes, meaning no cross-resistance. Predominantly bacteriostatic against staphylococci and enterococci, bactericidal against streptococci. Take with or without food. Avoid tyramine-rich foods during therapy (aged cheeses, fermented meats, wines, soy sauce) — linezolid is a weak reversible MAO inhibitor. Complete the full prescribed course.
Critical Warnings — Diabetic Patients
- Metformin — withhold during linezolid: Both linezolid and metformin impair mitochondrial function through different mechanisms — concurrent use increases lactic acidosis risk significantly. Withhold metformin for the entire duration of linezolid therapy; use insulin or alternative OHA. Inform your prescribing physician about metformin use.
- SSRIs/SNRIs — serotonin syndrome risk: Linezolid's MAO inhibitory activity combined with serotonergic drugs (sertraline, fluoxetine, duloxetine, venlafaxine — commonly used for diabetic neuropathic pain or depression) can cause serotonin syndrome. Physician must assess before prescribing linezolid.
