Overview
Arlin Oral Suspension contains Linezolid in liquid form — providing the same oxazolidinone antibiotic as the tablet and IV formulations in a suspension suitable for children, neonates, and adults who cannot swallow tablets. Linezolid is the only antibiotic in the oxazolidinone class with reliable activity against MRSA, VRE, and penicillin-resistant S. pneumoniae, and its near-complete oral bioavailability (~100%) makes the oral suspension therapeutically equivalent to IV linezolid. The oral suspension is the standard formulation for paediatric patients and is weight-dosed — allowing precise dose calculation across the paediatric age range.

Dosage and Administration

Shake gently before each use. Measure dose accurately using the oral syringe provided. Can be taken with or without food. Avoid tyramine-rich foods. Do not mix with other medications in the same measuring vessel. Store reconstituted suspension at room temperature and use within the period stated on the label (typically 21 days).
Critical Warnings for Diabetic Patients and Carers
- Metformin interaction — withhold for duration of linezolid: In adults on metformin, linezolid's mitochondrial inhibition is additive — lactic acidosis risk. Suspend metformin throughout linezolid course. For paediatric patients with type 1 diabetes, coordinate insulin management with their endocrinologist during infection and antibiotic therapy.
- SSRI/SNRI interaction — serotonin syndrome: Disclose all medications including antidepressants before prescribing linezolid.
- Paediatric type 1 diabetes monitoring: Infection with MRSA causes significant hyperglycaemia in children with type 1 diabetes — more frequent blood glucose monitoring and often increased insulin requirements during antibiotic treatment.
- Peripheral neuropathy monitoring: Report any new tingling, numbness, or weakness in hands or feet during therapy — particularly relevant in older children or adults with pre-existing diabetic neuropathy.
- FBC monitoring: Platelet count weekly for courses exceeding 14 days.
Side Effects
GI symptoms common (nausea, diarrhoea, vomiting — take with food to reduce). Headache common. Thrombocytopenia (weekly FBC for prolonged courses). Elevated LFTs. Skin rash (uncommon). Serotonin syndrome with serotonergic drugs. Lactic acidosis (rare; risk increased with concurrent metformin). Peripheral and optic neuropathy with prolonged use (>28 days).
Storage
Store unreconstituted powder below 25°C. Once reconstituted as suspension: store at room temperature — do NOT refrigerate. Use within 21 days of reconstitution. Shake gently before each dose. Keep out of reach of children. Discard remaining suspension after course is complete.
