Overview
Axodin 180mg is the highest-strength formulation in Beximco's Axodin range, containing Fexofenadine Hydrochloride 180 mg. This dose is the international standard for treating chronic idiopathic urticaria (CIU) — a persistent skin condition characterised by recurrent hives (wheals), intense itching, and sometimes angioedema (swelling), lasting more than 6 weeks without a clearly identified external cause. At 180mg once daily, fexofenadine provides superior H1 blockade to prevent the mast-cell histamine release underlying CIU. It remains non-sedating even at this higher dose, making it first-line therapy for CIU per international guidelines (EAACI/WAO).
Axodin Range — Dose Guide
| Strength | Dose | Frequency | Licensed For |
|---|---|---|---|
| Axodin 60mg | 60 mg | Twice daily | Seasonal allergic rhinitis |
| Axodin 120mg | 120 mg | Once daily | Seasonal allergic rhinitis |
| Axodin 180mg (this) | 180 mg | Once daily | Chronic idiopathic urticaria (CIU) |
What is Chronic Idiopathic Urticaria (CIU)?
CIU (also called chronic spontaneous urticaria) is a debilitating skin condition where itchy raised wheals (hives) appear spontaneously on the skin without an identifiable external trigger (unlike allergic urticaria from food or insect stings). Episodes recur for ≥6 weeks, often for months or years. Histamine released from skin mast cells is the primary driver. CIU significantly impairs quality of life — sleep disturbance from nocturnal pruritus is common. Second-generation antihistamines at standard-to-high doses (fexofenadine 180mg OD) are the first-line treatment per all international guidelines.
Indications
- Chronic Idiopathic Urticaria (CIU) — recurrent hives for >6 weeks; no identifiable external cause
- Severe Seasonal Allergic Rhinitis — when 120mg OD is insufficient
- Allergic Urticaria — hives from food, drugs, or other identified allergens (adjunct)
- Angioedema (histamine-mediated) — adjunct antihistamine; note: hereditary angioedema requires different treatment
Dosage & Administration
| Indication | Dose | Frequency |
|---|---|---|
| Chronic Idiopathic Urticaria (adults ≥12 yr) | 180 mg | Once daily |
| Severe Seasonal Allergic Rhinitis (adults) | 180 mg | Once daily (off-label; physician-directed) |
| Renal impairment (severe) | 60 mg once daily — use lowest effective dose | — |
How to take: Swallow with a glass of water only (not fruit juice). Take at the same time each day. Can be taken with or without food. Never take with grapefruit, orange, or apple juice — fruit juices reduce fexofenadine absorption significantly.
Special Note for Diabetic Patients
Fexofenadine 180mg has no effect on blood glucose, insulin secretion, or glycaemic control. Patients with diabetes who develop chronic urticaria can safely take Axodin 180mg. The antihistamine does not interact with commonly prescribed diabetes medications. Some older antihistamines (promethazine, chlorphenamine) cause sedation and appetite stimulation — fexofenadine avoids both of these concerns.
Side Effects
- Common: Headache (4.2% vs 3.8% placebo), nausea (1.4%), drowsiness (1.3% — minimal at 180mg), dizziness
- Uncommon: Fatigue, insomnia, nervousness, back pain
- Very rare: Hypersensitivity reactions — rash, urticaria (paradoxical), dyspnoea, anaphylaxis
- Fexofenadine 180mg remains essentially non-sedating — not classified as causing drowsiness at approved doses
Storage
- Store below 30°C in a dry place, protected from light. Keep out of reach of children.
FAQ
Q1: How long do I need to take Axodin 180mg for chronic urticaria?
CIU treatment duration varies. Most guidelines recommend continuous daily antihistamine therapy for at least 3–6 months before attempting dose reduction. Some patients achieve remission within months; others require long-term maintenance therapy. Your dermatologist or allergist will assess your response at regular intervals and guide dose tapering.
Q2: Can Axodin 180mg completely stop my hives?
In many patients, fexofenadine 180mg OD achieves complete or near-complete control of urticaria symptoms. In non-responders or partially controlled patients, physicians may prescribe doses up to 4× the licensed dose (720mg/day, off-label) or add omalizumab (anti-IgE biologic). Discuss with your dermatologist if 180mg does not adequately control symptoms.
Q3: Is Axodin 180mg safe to take every day for months?
Yes. Fexofenadine has an excellent long-term safety profile. It does not cause tachyphylaxis (loss of effect over time), hepatotoxicity, or cardiac arrhythmias. It can be taken continuously for the duration required to manage CIU.
Q4: Can I take Axodin 180mg with antibiotic erythromycin?
Erythromycin and ketoconazole can increase fexofenadine plasma concentrations, but clinical studies have shown this does not produce any significant safety concerns at therapeutic doses. Inform your physician about all medications you are taking.
