What is Bisopro?

Bisopro (Bisoprolol Fumarate) is a highly selective beta-1 adrenoceptor blocker manufactured by Incepta Pharmaceuticals Ltd., used in the treatment of hypertension, angina, and moderate to severe heart failure.

Indications

Indicated in hypertension, angina, and moderate to severe heart failure. Not recommended for emergency treatment of hypertensive crises.

Pharmacology

Bisoprolol is the most beta-1 selective blocker available, selectively blocking beta-1 adrenergic receptors in the heart and vascular smooth muscle, reducing heart rate and cardiac output and decreasing arterial hypertension, without adversely affecting lipid metabolism.

Dosage & Administration

Adults: usual starting dose 5 mg once daily (alone or with a diuretic); may be increased to 10 mg, then up to 20 mg once daily at 2-week intervals.
Hepatic/renal impairment: initial daily dose of 5 mg, with cautious titration.

Interaction

Should not be combined with other beta-blockers. Caution with catecholamine-depleting drugs, clonidine, calcium channel blockers (verapamil, diltiazem), and antiarrhythmics such as disopyramide.

Contraindications

Cardiogenic shock, overt heart failure, second/third-degree AV block, right ventricular failure secondary to pulmonary hypertension, sinus bradycardia.

Side Effects

Fatigue, dizziness, headache, GI disturbances (nausea, vomiting, diarrhea, constipation), cold/numb extremities, muscle weakness or cramps, bronchospasm in susceptible patients.

Pregnancy & Lactation

Use during pregnancy only if benefit justifies risk to the fetus. Not known if excreted in human milk; discontinue nursing if use is considered essential.

Precautions & Warnings

Use caution in renal/hepatic impairment. Patients with history of severe anaphylactic reactions may be unresponsive to usual epinephrine doses while on beta-blockers.
