What is Bisopro-A?
Bisopro-A combines Bisoprolol Fumarate, a cardioselective beta-1 blocker, with Amlodipine Besilate, a dihydropyridine calcium channel blocker, manufactured by Incepta Pharmaceuticals Ltd.
Indications
Indicated for the treatment of hypertension as substitution therapy in patients adequately controlled with the individual components given concurrently at the same dose levels.
Pharmacology
Amlodipine inhibits transmembrane calcium ion influx into vascular smooth muscle and cardiac muscle, with greater effect on vascular smooth muscle, reducing peripheral vascular resistance and blood pressure. Bisoprolol Fumarate is a beta-1 selective adrenoceptor blocker lacking intrinsic sympathomimetic activity, generally not expected to affect airway resistance.
Dosage & Administration
One tablet once daily in patients whose blood pressure is adequately controlled with separately administered monocomponent products at the same doses as the fixed-dose combination.
Interaction
Not recommended with verapamil/diltiazem-type calcium antagonists or centrally-acting antihypertensives. Caution with CYP3A4 inhibitors/inducers, simvastatin, tacrolimus, cyclosporine, class I/III antiarrhythmics, NSAIDs, and other blood-pressure-lowering agents.
Contraindications
Acute/decompensated heart failure, outflow tract obstruction (e.g., aortic stenosis), cardiogenic shock, second/third-degree AV block, sick sinus syndrome, severe bradycardia/hypotension, severe bronchial asthma, severe peripheral arterial occlusive disease, untreated phaeochromocytoma, hypersensitivity to either component.
Side Effects
Dizziness, headache, somnolence, palpitations, flushing, cold/numb extremities, GI complaints, edema, fatigue. Less commonly insomnia, mood changes, bradycardia, hypotension, dyspnea, bronchospasm.
Pregnancy & Lactation
Not recommended during pregnancy and lactation.
Precautions & Warnings
Use caution in heart failure, hepatic impairment, and obstructive airway disease. Must not be stopped abruptly, especially in ischemic heart disease. Caution in diabetes, peripheral arterial disease, and psoriasis.
