Overview
Cefotax 500mg contains Cefotaxime Sodium 500mg — a third-generation cephalosporin with broad-spectrum bactericidal activity against Gram-positive and Gram-negative bacteria. Cefotaxime provides excellent tissue penetration including the blood-brain barrier, making it a first-line choice for bacterial meningitis and serious systemic infections. The 500mg strength is used for moderate infections in adults and weight-appropriate dosing in children. It is particularly important in diabetic patients with severe skin, urinary tract, respiratory, or foot infections requiring parenteral antibiotic therapy.
Indications
- Pneumonia — community-acquired and hospital-acquired
- Bacterial meningitis — Gram-negative and streptococcal
- Septicaemia / bacteraemia
- Complicated UTI and pyelonephritis
- Intra-abdominal infections (with metronidazole)
- Diabetic foot infections — moderate to severe with Gram-negative involvement
- Neonatal sepsis and paediatric severe infections

Reconstitute with sterile water per hospital protocol. IV over 3–5 minutes or infusion over 20–60 minutes. Reduce dose in renal impairment (CrCl <20 mL/min). Always take wound/blood cultures before starting antibiotics.
Diabetic Patient Notes
- Infection causes profound hyperglycaemia — intensive glucose monitoring and IV insulin infusion standard during hospitalisation
- Check eGFR — dose reduction required in diabetic nephropathy
- Polymicrobial diabetic foot infections may require additional agents (metronidazole for anaerobes, anti-MRSA agent if MRSA suspected)
Side Effects and Contraindications
Injection site reactions, GI upset, and hypersensitivity reactions (rash) are most common. Anaphylaxis is rare (~1–2% cross-reactivity with penicillin allergy). C. difficile colitis uncommon. Contraindicated in cephalosporin hypersensitivity. Use with caution in severe penicillin allergy.
Storage
Store below 25°C, protected from light. Use reconstituted solution within 6 hours at room temperature or 24 hours refrigerated.
