Overview

Cotrim DS contains Sulphamethoxazole + Trimethoprim (Co-trimoxazole) in a 800 mg + 160 mg tablet, manufactured by Square Pharmaceuticals PLC. It is used for the treatment of urinary tract infections, respiratory tract infections (including Pneumocystis jirovecii pneumonia), typhoid fever, shigellosis, and other infections caused by susceptible organisms.

Dosage and Administration

Adults: one tablet twice daily for 7–14 days depending on infection type; DS (double-strength) used for more severe infections or urinary tract infections. Take with food and plenty of water.

Interaction

May enhance anticoagulant effect of warfarin. Increases methotrexate toxicity. Phenytoin levels may increase. Reduces efficacy of cyclosporin. May cause hyperkalaemia with ACE inhibitors or potassium-sparing diuretics.

Contraindications

Contraindicated in patients with severe hepatic or renal impairment, megaloblastic anaemia due to folate deficiency, hypersensitivity to sulphonamides or trimethoprim, and during the first trimester and near term of pregnancy.

Side Effects

Nausea, vomiting, rash (including severe reactions such as Stevens-Johnson syndrome), haematological disorders, and crystalluria. Renal function may be impaired.

Pregnancy and Lactation

Avoid during the first trimester (teratogenic risk of trimethoprim) and near term (risk of neonatal jaundice). Not recommended during breastfeeding.

Precautions and Warnings

Maintain adequate fluid intake to prevent crystalluria. Monitor full blood count and renal function with prolonged use. Use with caution in elderly and folate-deficient patients.
