Indications

Cran-B is indicated for:

• Prevention and treatment of urinary tract infections (UTIs) — particularly recurrent cystitis in women
• Kidney stone management — reducing incidence of stone formation at higher doses

Pharmacology

Cranberry (Vaccinium macrocarpon) is a North American medicinal plant with well-established urological benefits. Its primary mechanism of action involves inhibiting bacterial adhesion to uroepithelial surfaces — the key initiating event in urinary tract infection pathogenesis.

Cranberry contains proanthocyanidins (specifically type-A PACs) that block the fimbriae of uropathogenic Escherichia coli (UPEC), preventing them from adhering to the urothelial lining of the bladder and urethra. Without successful adhesion, bacteria cannot colonise and are flushed out in the urine stream. This mechanism is distinct from antibiotic action and does not contribute to antimicrobial resistance.

At higher doses, cranberry also acts as a urinary antiseptic, reducing the urinary pH and inhibiting stone-forming mineral precipitation. Clinical studies demonstrate up to 65% reduction in UTI incidence with regular cranberry supplementation, leading to significantly reduced need for repeated antibiotic courses and fewer episodes of recurrent cystitis.

Cranberry also enhances vitamin B12 absorption, which is particularly beneficial for patients on proton pump inhibitors (e.g., omeprazole) who are at risk of B12 deficiency.

Dosage & Administration

Capsules: 1–3 capsules daily, or as directed by the physician.

Best taken with a full glass of water. May be taken with or without food.

Contraindications

• Renal insufficiency (potential for oxalate accumulation)
• Persons with a predisposition to uric acid or calcium oxalate kidney stones (use with caution at high doses)

Side Effects

Generally well tolerated at recommended doses. At high doses, occasional mild gastrointestinal upset or diarrhoea may occur.

Drug Interactions

• No known interactions with antibiotics or most medications
• Enhances vitamin B12 absorption — beneficial in patients on omeprazole or other PPIs
• Theoretical interaction with warfarin at very high doses — monitor INR if relevant
