Overview
Diliner DR 60 mg Capsule contains Duloxetine Hydrochloride, a serotonin-norepinephrine reuptake inhibitor (SNRI) manufactured by Square Pharmaceuticals PLC. Duloxetine inhibits the reuptake of both serotonin and norepinephrine, and is used for the treatment of major depressive disorder (MDD), generalised anxiety disorder (GAD), diabetic peripheral neuropathic pain, fibromyalgia, and chronic musculoskeletal pain.
Dosage and Administration
Adults: for MDD/GAD — typically 60 mg once daily (starting at 30 mg for 1 week may improve tolerability). For neuropathic pain — 60 mg once daily. Maximum 120 mg/day in divided doses. Swallow whole; do not crush or chew. Take at the same time each day.
Interaction
Serious interaction with MAOIs — contraindicated; allow 14-day washout. Risk of serotonin syndrome with other serotonergic drugs (triptans, SSRIs, tramadol, lithium). Duloxetine is a moderate CYP2D6 inhibitor — may increase levels of TCA antidepressants, antipsychotics, and other CYP2D6 substrates. NSAIDs and anticoagulants increase bleeding risk.
Contraindications
Contraindicated within 14 days of MAOI use, in uncontrolled narrow-angle glaucoma, severe hepatic impairment (cirrhosis), and known hypersensitivity to duloxetine.
Side Effects
Nausea (most common, especially early), dry mouth, dizziness, constipation, fatigue, hyperhidrosis, and insomnia. Blood pressure elevation with long-term use. Discontinuation syndrome if stopped abruptly.
Pregnancy and Lactation
Avoid during pregnancy if possible; potential risk of neonatal complications. Excreted in breast milk — use with caution during breastfeeding.
Precautions and Warnings
Do not stop abruptly — taper gradually over several weeks to avoid discontinuation syndrome (dizziness, nausea, paraesthesia). Monitor blood pressure regularly. Monitor for suicidal ideation especially in early treatment.
