Description
DUOCARD (Cilnidipine) is a dihydropyridine (DHP) type of calcium channel antagonist which inhibits sympathetic Ntype calcium channels in addition to vascular L-type calcium channels. The L-type calcium channel blockade by cilnidipine affects predominantly vascular smooth muscle, thereby producing vasodilation of peripheral resistance vessels and coronary arteries. The blockade of N-type Ca2+ channels affects predominantly peripheral nerve endings of sympathetic neurons, thereby dilating blood vessels by lowering plasma catecholamine levels. Thus

Composition
DUOCARD 5: Each film-coated tablet contains Cilnidipine INN 5 mg.
DUOCARD 10: Each film-coated tablet contains Cilnidipine INN 10 mg.
Indication
For the management of Hypertension.
Dosage and Administration
The recommended adult oral dosage of DUOCARD is 5-10 mg once daily. The dosage can be increased up to 20 mg, if needed or as directed by the Physician
Contraindications
Cilnidipine is contraindicated in cardiogenic shock, recent MI or acute unstable angina, severe aortic stenosis. Precautions Hypotension, poor cardiac reserve, heart failure. Sudden withdrawal may exacerbate angina. Discontinue in patients who experience ischemic pain following administration.
Use in Pregnancy & Lactation
In Pregnancy: No specific information about USFDA pregnancy category. Caution should be exercised during Cilnidipine use in pregnancy. In Lactation: Nursing mothers should consult a physician before taking Cilnidipine.
Adverse Reactions
Dizziness, flushing, headache, hypotension, peripheral edema, tachycardia, palpitations, GI disturbances, increased micturition frequency, lethargy, eye pain, depression, ischemic chest pain, cerebral or myocardial ischemia, transient blindness, rashes, fever, abnormal liver function, gingival hyperplasia, myalgia, tremor and impotence.
Drug Interactions
Other antihypertensives, aldesleukin, antipsychotics that cause hypotension, may modify insulin and glucose responses, quinidine; carbamazepine, phenytoin, rifampicin, cimetidine, erythromycin.
