Overview

Efaxim 550 mg Tablet contains Rifaximin in a higher-dose formulation, manufactured by Square Pharmaceuticals PLC. The 550 mg tablet is the standard dose used for reduction of risk of overt hepatic encephalopathy (HE) recurrence in adults, in combination with lactulose. Rifaximin acts locally in the gut to reduce ammonia-producing colonic bacteria, thereby lowering blood ammonia levels in patients with hepatic encephalopathy.

Dosage and Administration

Hepatic encephalopathy: 550 mg twice daily. Take with or without food. Use in combination with lactulose as prescribed. Ongoing maintenance dosing as recommended by gastroenterologist or hepatologist.

Interaction

Minimal systemic drug interactions due to negligible absorption. In severe hepatic impairment, some systemic absorption may occur; cyclosporin may increase exposure to rifaximin. Monitor patients on concomitant hepatotoxic drugs.

Contraindications

Contraindicated in diarrhoea with fever or blood, systemic infections requiring systemic antibiotics, and known hypersensitivity to rifaximin or any rifamycin.

Side Effects

Generally well tolerated. Reported effects include peripheral oedema, nausea, dizziness, headache, and muscle spasms.

Pregnancy and Lactation

Limited data in pregnancy — avoid unless benefit outweighs risk. Not recommended during breastfeeding.

Precautions and Warnings

Intended for maintenance prevention of HE recurrence — not for acute HE management in hepatic coma. Continue lactulose therapy alongside. Alert physician if new neurological symptoms or fever develop.

Storage Conditions

Store below 25°C in a cool, dry place away from moisture.

Frequently Asked Questions

What is the main clinical use of rifaximin 550 mg vs 200 mg?
The 200 mg tablet is used for travellers' diarrhoea and SIBO (short courses), while the 550 mg is specifically used twice daily for long-term maintenance to prevent recurrence of hepatic encephalopathy in patients with chronic liver disease.

How does rifaximin reduce hepatic encephalopathy?
Ammonia-producing bacteria in the colon are major contributors to the elevated blood ammonia seen in HE; rifaximin suppresses these colonic bacteria locally, reducing ammonia production and helping to prevent episodes of confusion and neurological deterioration.
