Overview

Eromycin DS 500 mg Tablet contains Erythromycin 500 mg (Double Strength), manufactured by Square Pharmaceuticals PLC. The DS (double strength) formulation provides a higher dose for adult use and for more severe infections. Erythromycin inhibits bacterial protein synthesis via 50S ribosomal binding and is used for respiratory tract infections, whooping cough, skin and soft tissue infections, chlamydial urogenital infections, Campylobacter gastroenteritis, and as a penicillin substitute in hypersensitive patients.

Dosage and Administration

Adults: 500 mg every 6 hours (2 g/day), or 500 mg every 12 hours for mild infections, as prescribed. Take on an empty stomach (30 minutes before meals) for better absorption, or with food if GI intolerance occurs. Maximum 4 g/day in severe infections.

Interaction

Major CYP3A4 inhibitor: erythromycin significantly increases levels of statins (risk of myopathy/rhabdomyolysis), warfarin (increased bleeding), digoxin, theophylline, cyclosporin, and carbamazepine. Contraindicated with cisapride, pimozide, and ergotamine. QT-prolonging drug interactions.

Contraindications

Contraindicated in severe hepatic disease (estolate form), known macrolide hypersensitivity, concurrent use with QT-prolonging drugs, cisapride, pimozide, and ergotamine derivatives.

Side Effects

GI side effects (nausea, vomiting, cramping) most common; cholestatic jaundice (estolate); reversible hearing loss at high doses; cardiac arrhythmia (QT prolongation) — especially at high doses or in patients with cardiac disease.

Pregnancy and Lactation

Generally considered safe in pregnancy (base form). Compatible with breastfeeding. Avoid estolate form in pregnancy.

Precautions and Warnings

Complete the full prescribed course. Inform physician and pharmacist of all medications before starting. Monitor for signs of jaundice. Take at regular intervals.
