Femastin Cream 15 gm

Overview

Femastin 0.1% Vaginal Cream contains Estriol 0.1% (1 mg per gram) for topical vaginal application, manufactured by Square Pharmaceuticals PLC. The vaginal route provides targeted local oestrogen delivery to the vaginal epithelium and surrounding urogenital tissues with minimal systemic absorption compared to oral oestrogen. Used for urogenital atrophy in post-menopausal women: vaginal dryness, itching, burning, dyspareunia (painful intercourse), recurrent vaginal infections, and urinary urgency or stress incontinence due to urogenital atrophy.

Dosage and Administration

Initial: one application (0.5 g = 0.5 mg estriol) intravaginally daily for 2–3 weeks; maintenance: 0.5 g twice weekly, or as directed. Use the graduated applicator provided. The lowest effective dose should be used.

Interaction

Systemic absorption is low; interactions similar to oral estriol are possible if significant absorption occurs, especially with anticoagulants and enzyme inducers.

Contraindications

Contraindicated in oestrogen-sensitive malignancies, undiagnosed genital bleeding, active thromboembolism, and known hypersensitivity to estriol.

Side Effects

Local: mild vaginal burning, pruritus on initial application. Systemic: rare at therapeutic vaginal doses. Endometrial stimulation is minimal at recommended vaginal doses.

Pregnancy and Lactation

Contraindicated during pregnancy. Avoid during breastfeeding as it may affect milk composition or suppress lactation.

Precautions and Warnings

Regular gynaecological review recommended with any oestrogen therapy. For women with an intact uterus on prolonged use, physician assessment of endometrial safety is advised. Use the minimum effective dose.
