Flugal Oral Suspension contains Fluconazole 50 mg per 5 ml, providing a convenient liquid formulation suitable for patients who have difficulty swallowing capsules, including children and elderly patients. Fluconazole is a synthetic triazole antifungal that inhibits cytochrome P450-dependent 14α-lanosterol demethylase, disrupting ergosterol biosynthesis in fungal cell membranes. This formulation is effective for oropharyngeal candidiasis, tinea infections, pityriasis versicolor, and onychomycosis in patients requiring liquid dosing.

Dosage and Administration

Shake well before use. Oropharyngeal Candidiasis: Adults — 200 mg (20 ml) on day 1, then 100 mg (10 ml) daily for at least 14 days. Children — 6 mg/kg on day 1, then 3 mg/kg daily. Esophageal Candidiasis: Adults — 200 mg day 1, then 100 mg daily for minimum 21 days. Vaginal Candidiasis: 150 mg (15 ml) single dose. Tinea infections: 150 mg (15 ml) once weekly for 4–6 weeks. Onychomycosis: 150 mg (15 ml) once weekly for 6–12 months. For children, maximum dose 400 mg/day. May be taken with or without food.

Interaction

Warfarin: increased prothrombin time — monitor INR closely. Sulphonylureas: increased hypoglycaemic effect. Hydrochlorothiazide: raises fluconazole plasma levels by approximately 40%. Phenytoin: plasma levels may increase significantly. Rifampicin: reduces fluconazole AUC and half-life. Cyclosporin: elevated plasma levels — monitor. Theophylline: decreased clearance. Cisapride and terfenadine: co-administration contraindicated.

Contraindications

Contraindicated in patients with known hypersensitivity to fluconazole or other azole/triazole antifungal agents. Concomitant use with terfenadine or cisapride is contraindicated. Patients with rare hereditary conditions such as fructose intolerance should consult their physician before use as the suspension may contain excipients.

Side Effects

Common: nausea, abdominal discomfort, diarrhoea, flatulence. Rash may develop; discontinue if bullous or exfoliative skin reactions occur. Rare but serious: anaphylaxis, hepatotoxicity, Stevens-Johnson syndrome. Transient liver enzyme elevations may occur, particularly with prolonged use.

Pregnancy and Lactation

Fluconazole is not recommended during pregnancy and is contraindicated in women of childbearing potential not using effective contraception. It passes into breast milk; breastfeeding should be avoided during treatment.
