Indications

Fructin is used intravenously as a carbohydrate nutrient. It is converted to liver glycogen and metabolized more rapidly than dextrose without requiring insulin, and thus may be used in diabetic patients. It is indicated in patients requiring fluid replacement and caloric feeding and is a better nitrogen sparer than glucose.

Pharmacology

Fructose is a monosaccharide carbohydrate that is metabolized in the liver via the fructose-1-phosphate pathway, independent of insulin. This allows it to be used as an energy source even in insulin-deficient states. It provides calories and spares protein by reducing nitrogen loss.

Dosage & Administration

The volume and rate of infusion depends upon the requirements of the patient and the judgement of the attending physician. The rate of infusion should not exceed 0.5-1 g/kg body weight per hour. The usual recommended flow rate for adults is 30-75 drops per minute by intravenous infusion.

Side Effects

Intravenous administration may cause lactic acidosis and hyperuricaemia. Rapid infusion results in facial flushing, abdominal pain and sweating.

Precautions & Warnings

Should not be given to patients with hereditary fructose intolerance. Should be given with caution to patients with impaired kidney function or severe liver damage. Infusion should be immediately discontinued if rigor arises. Do not use if the solution is cloudy, contains particles, or after expiry date.

Contraindications

Contraindicated in certain inborn errors of fructose metabolism, methyl alcohol poisoning, hyperuricaemia, liver failure, and lactic acidosis.

Storage Conditions

Keep below 30°C temperature, away from light and moisture. Keep out of the reach of children.
