Overview

Genacyn 0.1% Ointment (Gentamicin Sulfate) is a topical antibacterial preparation for the treatment and prevention of primary and secondary skin infections caused by susceptible gram-negative and gram-positive organisms. Gentamicin inhibits bacterial protein synthesis by binding irreversibly to the 30S ribosomal subunit. The ointment base provides a protective, occlusive layer that maintains moisture at the wound or infection site, enhancing drug penetration and promoting healing. It is indicated for infected burns, impetigo, folliculitis, furunculosis, infected eczema, wound infections, pyoderma, and infected lacerations or abrasions.

Dosage and Administration

Apply a thin layer of ointment to the affected skin area three to four times daily. For infected burns or large wounds, clean the area gently before each application. The ointment may be covered with a light non-occlusive dressing if necessary. Continue treatment for the full prescribed course (typically 7–14 days). Wash hands thoroughly before and after application. Do not apply to the eyes.

Interaction

Systemic absorption of topical gentamicin from intact skin is negligible. In cases of large-area application to broken or burnt skin, absorption may be sufficient to interact with concurrent systemic nephrotoxic or ototoxic agents. Concurrent systemic aminoglycosides or vancomycin should be used with caution if large areas are treated.

Contraindications

Contraindicated in known hypersensitivity to gentamicin or other aminoglycosides. Not for ophthalmic use (use Genacyn 0.3% eye drops for eye infections). Avoid use on large body surface areas for prolonged periods in patients with renal impairment.

Local reactions include mild burning, stinging, or skin irritation at the application site. Allergic contact dermatitis may occur in sensitised individuals. Prolonged use on extensive skin areas may rarely cause systemic effects (nephrotoxicity, ototoxicity). Overgrowth of resistant organisms or fungi may occur with extended use.

Systemic absorption from small-area topical application is minimal. Gentamicin ointment is generally considered safe for use during pregnancy and breastfeeding when applied to limited areas. Avoid extensive application during the first trimester. Consult a physician before use during pregnancy or lactation.

Precautions and Warnings

For external use only; avoid contact with eyes. Do not apply to large areas of broken skin for prolonged periods without medical supervision, as systemic absorption can occur. Reassess the infection if no improvement is seen after 7 days. Prolonged use may allow resistant organisms or fungi to proliferate — if this occurs, discontinue and seek alternative treatment. Discard the tube 4 weeks after opening.
