Overview
I-PENAM I.V 500mg contains Meropenem 500mg — a carbapenem antibiotic representing the broadest-spectrum class of beta-lactams. Carbapenems are reserved for the most serious, resistant, or polymicrobial infections where standard antibiotics are ineffective. Meropenem covers ESBL-producing E. coli and Klebsiella, Pseudomonas aeruginosa, virtually all Gram-negative and Gram-positive aerobic bacteria, and anaerobes — making it uniquely suitable for empirical therapy in critically ill patients and definitive treatment of culture-confirmed carbapenem-susceptible resistant infections.
Indications
- ESBL-producing Gram-negative infections (UTI, pneumonia, bacteraemia)
- Hospital-acquired and ventilator-associated pneumonia (HAP/VAP)
- Febrile neutropenia — empirical monotherapy
- Complicated intra-abdominal infections
- Severe diabetic foot infections — deep tissue, bone, polymicrobial
- Septicaemia and septic shock — broadest empirical coverage
- Pseudomonas aeruginosa — respiratory, urinary, bloodstream

Dilute in 50–250 mL NS; infuse IV over 15–30 minutes (or 3–4 hour extended infusion for resistant organisms). Use within 1 hour of reconstitution at room temperature.
Diabetic Patient Notes
- Mandatory eGFR check — diabetic nephropathy requires dose reduction
- ESBL UTIs/bacteraemia in diabetics: meropenem is definitive treatment of choice
- Severe diabetic foot infections with systemic signs require ICU-level care and meropenem empirically
- Intensive glucose monitoring during sepsis — IV insulin infusion for ICU patients
- De-escalate to narrower antibiotics as soon as culture results allow
Side Effects and Contraindications
Common: GI upset, elevated LFTs, injection site phlebitis. Uncommon: rash, C. difficile colitis. Rare: anaphylaxis (~1% penicillin cross-reactivity), seizures (much less than imipenem). Contraindicated in meropenem/carbapenem hypersensitivity.
Storage
Store below 25°C, protected from light. Use within 1 hour at room temperature after reconstitution. Keep out of reach of children.
