Indications

Ibrucent (Ibrutinib) is a Bruton's Tyrosine Kinase (BTK) inhibitor indicated for:

• Mantle Cell Lymphoma (MCL): In patients who have received at least one prior therapy.
• Chronic Lymphocytic Leukemia/Small Lymphocytic Lymphoma (CLL/SLL): Including patients with 17p deletion.
• Waldenström's Macroglobulinemia (WM).
• Marginal Zone Lymphoma (MZL): In patients requiring systemic therapy who have received at least one prior anti-CD20-based therapy.

Pharmacology

Ibrutinib is a small-molecule inhibitor of BTK. It forms a covalent bond with a cysteine residue in the BTK active site, leading to inhibition of BTK enzymatic activity. BTK's role in B-cell surface receptor signaling results in activation of pathways necessary for B-cell trafficking, chemotaxis, and adhesion. Ibrutinib inhibits malignant B-cell proliferation and survival.

Dosage & Administration

Administer orally once daily at approximately the same time each day. Swallow capsules whole with water — do not open, break, or chew.

• MCL & MZL: 560 mg (four 140 mg capsules) once daily until disease progression or unacceptable toxicity.
• CLL/SLL & WM: 420 mg (three 140 mg capsules) once daily until disease progression or unacceptable toxicity.

Contraindications

Hypersensitivity to Ibrutinib or any excipient of the formulation.

Side Effects

Important adverse reactions include hemorrhage, infections, cytopenias, atrial fibrillation, hypertension, second primary malignancies, and tumor lysis syndrome. Additional reactions: diarrhea and visual disturbance.

Drug Interactions

• CYP3A Inhibitors: Avoid co-administration with strong/moderate CYP3A inhibitors. If a moderate inhibitor must be used, reduce Ibrutinib dose.
• CYP3A Inducers: Avoid co-administration with strong CYP3A inducers.

Pregnancy & Lactation

Can cause fetal harm based on animal studies. Not recommended during pregnancy; women of reproductive potential should use effective contraception. No information available on presence in human breast milk.

Use in Special Populations

Pediatric: Safety and efficacy not established. Geriatric: No overall differences in effectiveness vs. younger patients; pneumonia and anemia may be more frequent in older patients. Hepatic impairment: Not recommended in moderate/severe hepatic impairment; monitor for toxicity in mild impairment.
