Overview

Lebac 500 mg Capsule contains Cephradine, a semisynthetic first-generation cephalosporin antibiotic manufactured by Square Pharmaceuticals PLC. It exerts bactericidal action by inhibiting bacterial cell wall synthesis, leading to lysis of susceptible organisms. Cephradine is active against a broad range of Gram-positive and Gram-negative bacteria including penicillinase-producing staphylococci and many ampicillin-resistant strains.

Lebac 500 mg is indicated for upper and lower respiratory tract infections, urinary tract infections, and skin and soft tissue infections caused by susceptible organisms. The 500 mg strength is suited for moderate to more severe infections or where higher dosing is clinically required.

Dosage and Administration

Adults: Urinary tract infections: 500 mg four times daily or 1 g twice daily. Respiratory tract infections and skin/soft tissue infections: 250–500 mg four times daily or 500 mg–1 g twice daily. Maximum daily dose: 4 g. For chronic or severe infections, up to 1 g four times daily may be necessary. Treatment should continue at least 2–3 days after symptom resolution; streptococcal infections require at least 10 days.

Children: 25–50 mg/kg/day in 2–4 equally divided doses (paediatric dosage should not exceed adult doses). Renal impairment: Dose adjustment required based on creatinine clearance.

Interaction

Concomitant use with aminoglycosides or loop diuretics (furosemide, ethacrynic acid) may increase the risk of nephrotoxicity. Probenecid reduces renal excretion of Cephradine, increasing plasma concentrations. Cephradine may produce false-positive urine glucose results with Benedict's or Fehling's tests and Clinitest tablets; use enzyme-based tests (Clinistix, Diastix) for accurate results.

Contraindications

Contraindicated in patients with known or suspected hypersensitivity to cephalosporins. Use with caution in penicillin-allergic patients due to potential cross-sensitivity between beta-lactam antibiotics.

Side Effects

Most commonly: gastrointestinal disturbances (nausea, vomiting, diarrhea, abdominal pain, heartburn) and hypersensitivity reactions (skin rash, urticaria, joint pains, edema). Rare effects include blood dyscrasias (thrombocytopenia, agranulocytosis, aplastic anemia, hemolytic anemia), anaphylaxis, serum sickness-like reactions, transient hepatitis, cholestatic jaundice, reversible interstitial nephritis, and CNS effects (dizziness, nervousness). Candida overgrowth may occur with prolonged use.

Pregnancy and Lactation

Animal studies have not demonstrated teratogenicity, but safety during human pregnancy has not been fully established. Cephradine is excreted in breast milk; use with caution in lactating mothers. Patients should be cautioned about operating machinery due to the risk of dizziness.

Precautions and Warnings

Prolonged antibiotic use may cause superinfection with resistant organisms. Cross-reactivity with penicillins should be considered in allergic patients. Cephalosporins may cause a positive Coombs' test. Lebac capsules contain lactose and are unsuitable for patients with galactose intolerance, Lapp lactase deficiency, or glucose-galactose malabsorption. Renal function should be monitored in patients on high doses or with pre-existing renal impairment.
