What is Osartil?
Osartil contains Losartan Potassium, the first non-peptide orally active angiotensin II receptor blocker (ARB). It binds to the AT1 receptor in vascular smooth muscle, adrenal gland, kidneys, and heart, reducing vasoconstriction and aldosterone release to lower blood pressure.
Indications
Hypertension: Used alone or in combination with other antihypertensives (e.g. thiazide diuretics).
Renal Protection in Type-2 Diabetics with Proteinuria: Delays progression of renal disease in hypertensive type-2 diabetic patients with proteinuria (urinary albumin to creatinine ratio >300 mg/g).
Pharmacology
Losartan Potassium blocks the AT1 receptor, inhibiting the vasoconstrictive and aldosterone-secreting effects of angiotensin II, thereby lowering blood pressure and providing renal protective effects in diabetic nephropathy.
Dosage & Administration
Usual starting and maintenance dose is 50 mg once daily for most patients. If the antihypertensive effect is inadequate, 25 mg twice daily may be considered before increasing the dose. For intravascularly volume-depleted patients (e.g. on high-dose diuretics), a starting dose of 25 mg once daily is recommended. Total daily dose ranges from 25-100 mg, given once or twice daily. Patients over 75 years: lower starting dose of 25 mg once daily recommended.
Interaction
Rifampicin and fluconazole reduce active metabolite levels. Concomitant hydrochlorothiazide potentiates antihypertensive effect. Potassium-sparing diuretics, potassium supplements, or potassium-containing salt substitutes may increase serum potassium. Indomethacin may attenuate antihypertensive effect. Combined use with ACE inhibitors, other ARBs, NSAIDs, and thiazide diuretics increases risk of renal impairment.
Contraindications
Contraindicated in pregnancy and in patients hypersensitive to any component. Should not be administered with Aliskiren in diabetic patients.
Side Effects
Generally mild and transient: dizziness, diarrhea, nasal congestion, cough, upper respiratory infection. Less commonly: fatigue, oedema, abdominal pain, chest pain, nausea, headache, pharyngitis.
Pregnancy & Lactation
Pregnancy Category D. Risk to fetus increases if administered during the second or third trimester. Use during lactation should be decided based on risk-benefit, as excretion in human milk is unknown.
Precautions & Warnings
Use during second/third trimester reduces fetal renal function and increases fetal/neonatal morbidity and mortality. Symptomatic hypotension may occur in volume-depleted patients. Plasma concentration is significantly increased in cirrhotic patients; lower dose should be considered in hepatic impairment. Monitor renal function in patients with bilateral renal artery stenosis or stenosis to a solitary kidney. Should not be combined with potassium-sparing diuretics.
