What is Montair?
Montair is a selective and orally active leukotriene receptor antagonist used in the prophylaxis and chronic treatment of asthma and for relief of allergic rhinitis symptoms.
Indications
Montair is indicated for:
- Prophylaxis and chronic treatment of asthma
- Acute prevention of Exercise-Induced Bronchoconstriction (EIB)
- Relief of symptoms of Allergic Rhinitis (AR): Seasonal & Perennial Allergic Rhinitis
Pharmacology
Montelukast is a selective and orally active leukotriene receptor antagonist that inhibits the cysteinyl leukotriene receptor (CysLT1). Cysteinyl leukotrienes (LTC4, LTD4, LTE4) are products of arachidonic acid metabolism released from cells such as mast cells and eosinophils, and are correlated with the pathophysiology of asthma and allergic rhinitis, including airway edema, smooth muscle contraction, and inflammatory cellular activity.
Dosage & Administration
Adults and adolescents 15 years and older: Montelukast 10 mg tablet once daily.
Pediatric patients 6 to 14 years: Montelukast 5 mg tablet once daily.
Pediatric patients 2 to 5 years: Montelukast 4 mg tablet once daily.
No dosage adjustment is required in mild-to-moderate hepatic insufficiency, renal insufficiency, or in the elderly.
Interaction
No clinically important effects were seen on the pharmacokinetics of theophylline, prednisone, prednisolone, oral contraceptives, terfenadine, digoxin, or warfarin. Phenobarbital, a hepatic enzyme inducer, decreased Montelukast AUC by approximately 40%; clinical monitoring is advised when co-administered with potent CYP450 inducers such as phenobarbital or rifampin.
Contraindications
Contraindicated in patients hypersensitive to any component of this product.
Side Effects
Common: Diarrhoea, fever, GI discomfort, headache, nausea, vomiting, skin reactions, upper respiratory tract infection. Uncommon: Akathisia, anxiety, arthralgia, asthenia, abnormal behavior, depression, dizziness, drowsiness, dry mouth, oedema, sleep disorders. Rare: Angioedema, hallucination, hepatic disorders, memory loss, palpitations, tremor.
Pregnancy & Lactation
No adequate, well-controlled studies exist in pregnant women; Montelukast should be used during pregnancy only if clearly needed. Caution should be exercised when given to a nursing mother.
Precautions & Warnings
Not indicated for reversal of acute bronchospasm, including status asthmaticus; patients should have rescue medication available. Should not be abruptly substituted for inhaled or oral corticosteroids. Should not be used as monotherapy for exercise-induced bronchospasm. Aspirin-sensitive patients should continue to avoid aspirin/NSAIDs.
Overdose Effects
Reported overdose experiences include abdominal pain, somnolence, thirst, headache, vomiting and psychomotor hyperactivity. Supportive measures and clinical monitoring are recommended.
