Overview

Nitaxide contains Nitazoxanide 500mg — a broad-spectrum thiazolide antiprotozoal agent active against a wide range of intestinal protozoa, helminths, and anaerobic bacteria. It is the first-line treatment for Cryptosporidium parvum and Giardia lamblia infections and has activity against numerous other intestinal pathogens. Nitazoxanide is particularly relevant in diabetic patients, who have impaired immune defence against opportunistic intestinal parasites and are at greater risk of prolonged and severe diarrhoeal illness from Cryptosporidium — which can cause serious dehydration and glycaemic destabilisation.

Mechanism of Action

Nitazoxanide (and its active metabolite tizoxanide) inhibits the pyruvate:ferredoxin oxidoreductase (PFOR) enzyme — an anaerobic energy metabolism pathway essential to protozoa and anaerobic bacteria but absent in mammalian cells. This selective mechanism produces broad antimicrobial activity with minimal host cell toxicity. Tizoxanide also interferes with the electron transport chain of certain helminths.

Indications

• Cryptosporidium parvum — acute diarrhoea (especially in immunocompromised/diabetic patients)
• Giardia lamblia (Giardiasis) — intestinal infection, malabsorption syndrome
• Entamoeba histolytica — amoebiasis
• Intestinal helminths — ascaris, hookworm, strongyloides
• Clostridium difficile-associated diarrhoea (adjunctive)
• Rotavirus and norovirus gastroenteritis (studied; off-label)

Dosage and Administration

Take with food — food increases nitazoxanide absorption significantly and reduces GI side effects. Swallow whole. Do not crush or chew.

Diabetic Patient Guidance

• Cryptosporidium risk: Diabetic patients with poorly controlled blood sugar have impaired cellular immunity, making them susceptible to prolonged Cryptosporidium infection. Diarrhoea from Cryptosporidium in diabetics can cause severe dehydration and unpredictable glycaemic swings — seek treatment early.
• Glycaemic impact of diarrhoea: Acute diarrhoea reduces carbohydrate absorption and can cause hypoglycaemia in patients on sulfonylureas or insulin — monitor blood glucose closely during treatment and reduce doses if needed on advice of physician.
• Oral hypoglycaemics and GI illness: During active diarrhoea/vomiting, temporarily withhold metformin (risk of dehydration and lactic acidosis) and SGLT2 inhibitors (risk of DKA in volume-depleted state). Resume only when eating and drinking normally.
• Hydration: Maintain adequate hydration during diarrhoeal illness — dehydration worsens hyperglycaemia and increases the risk of AKI in patients with diabetic nephropathy.

Side Effects

Generally well tolerated. Common: abdominal pain, nausea, headache, urine discolouration (yellow-green — harmless, due to tizoxanide metabolite). Uncommon: diarrhoea paradoxically can worsen briefly at start of treatment. Rare: allergic reactions, elevated LFTs.

Contraindications and Precautions

Hypersensitivity to nitazoxanide. Hepatic impairment — use with caution; dose adjustment may be needed in severe liver disease. Pregnancy — limited data; use only if benefit outweighs risk. Breastfeeding — excreted in breast milk; caution advised.

Drug Interactions

Nitazoxanide is highly plasma protein bound (~99%) — theoretical displacement interactions with warfarin and other highly protein-bound drugs. Monitor anticoagulant effect if co-prescribed. No significant interactions with common diabetes medications.

Store below 25°C in a dry place. Protect from light and moisture. Keep out of reach of children.
