Overview

Nutrimin is a parenteral nutrition solution providing essential and non-essential Amino Acids, D-Sorbitol (as an alternative carbohydrate energy source), and Electrolytes for intravenous administration. It is indicated when enteral nutrition is not possible or insufficient and the patient requires parenteral protein and caloric supplementation. The inclusion of D-Sorbitol provides energy without requiring insulin for cellular uptake (sorbitol is converted to fructose, then glucose, in the liver) — making it particularly useful in diabetic patients or situations where glucose tolerance is compromised. Nutrimin is used in hospital settings for post-operative recovery, severe illness, and as a component of Total Parenteral Nutrition (TPN).

Indications

Critical Diabetic Patient Warnings

  • D-Sorbitol and diabetes: Although D-Sorbitol has lower glycaemic impact than glucose, it is not insulin-neutral — it is converted to fructose and then glucose in the liver. In patients with hepatic dysfunction (common in long-standing diabetes or NAFLD), sorbitol conversion may be impaired. Monitor blood glucose every 1–2 hours during infusion.
  • Hyperglycaemia during TPN: Parenteral nutrition is a major cause of hyperglycaemia in hospitalised diabetic patients. Insulin should be added to the TPN bag or administered via continuous IV infusion to target blood glucose 6–10 mmol/L (ICU) or 6–12 mmol/L (general ward).
  • Hepatic steatosis: Prolonged amino acid + carbohydrate TPN without lipids can cause hepatic steatosis — add lipid emulsion and cycle TPN where possible.
  • Refeeding syndrome: In severely malnourished diabetic patients, introduce TPN gradually and monitor phosphate, potassium, and magnesium — electrolyte shifts during refeeding can be life-threatening.
  • Electrolyte monitoring: Require daily electrolyte checks during TPN — particularly potassium (insulin drives K⁺ into cells during TPN glucose infusion) and phosphate.

Administration

Administer via peripheral or central venous catheter depending on osmolality. Inspect bag before use — do not use if cloudy or if precipitate visible. Administer at the prescribed rate using an infusion pump. Single use — discard unused portion. Aseptic technique essential throughout preparation and administration.

Side Effects

Metabolic: hyperglycaemia, hypokalaemia, hyperammonaemia (if hepatic dysfunction). Infusion-related: thrombophlebitis (peripheral), air embolism, catheter sepsis (central line). Metabolic: refeeding syndrome in malnourished patients. Hepatic: cholestasis and steatosis with prolonged use.

Storage

Store at controlled room temperature 15–25°C. Do not freeze. Protect from light. Do not use if outer packaging is damaged. Single use only.
