What is Pantonix Injection?

Pantonix Injection is an injectable proton pump inhibitor (PPI) containing Pantoprazole Sodium, indicated for patients unable to take oral medication. It covalently binds to the H+/K+-ATPase enzyme system on the gastric parietal cell, inhibiting both basal and stimulated gastric acid secretion for over 24 hours.

Indications

Pantonix is indicated where suppression of acid secretion has therapeutic benefit: peptic ulcer diseases, gastroesophageal reflux disease (GERD), ulcers induced by NSAIDs, eradication of Helicobacter pylori (with antibiotics), and Zollinger-Ellison Syndrome.

Pharmacology

Pantoprazole is a proton pump inhibitor that suppresses the final step in gastric acid production by covalently binding to the H+/K+-ATPase enzyme system at the surface of the gastric parietal cell, leading to inhibition of both basal and stimulated gastric acid secretion irrespective of the stimulus.

Dosage & Administration

Duodenal and gastric ulcer: 40 mg once daily for 7-10 days.
GERD with history of erosive esophagitis: 40 mg once daily for 7-10 days.
Prevention of rebleeding in peptic ulcer: IV 80 mg, followed by 8 mg/hour infusion for 72 hours.
Prophylaxis of acid aspiration: 80 mg IV every 12 hours for 24 hours, followed by 40 mg every 12 hours.
Long-term management of Zollinger-Ellison Syndrome: 80 mg IV every 12 hours, may increase to 80 mg every 8 hours if needed.
IV injection: Reconstitute with 10 ml 0.9% Sodium Chloride Injection; give slowly over 2-5 minutes. IV infusion: Reconstitute with 10 ml 0.9% Sodium Chloride, further dilute to 100 ml; infuse over approximately 15 minutes.

Interaction

No clinically significant drug interactions have been observed in clinical studies with Pantoprazole.

Contraindications

Contraindicated in patients with known hypersensitivity to any component of the formulation.

Side Effects

Generally well tolerated. Headache and diarrhoea are common; abdominal pain, flatulence, rash, insomnia, and hyperglycaemia occur rarely.

Pregnancy & Lactation

US FDA Pregnancy Category B. No adequate well-controlled studies exist in pregnant women; use only if clearly needed. Pantoprazole is excreted in human milk — decide whether to discontinue nursing or the drug.

Precautions & Warnings

Long-term therapy may lead to malabsorption of Vitamin B12 or increase risk of osteoporosis-related fractures. Use only freshly prepared solution; reconstituted solution may be stored at room temperature (up to 30°C) for a maximum of 4 hours.

Overdose Effects

No known symptoms of overdosage in humans reported. As Pantoprazole is highly protein bound, it is not readily dialyzable. Treatment should be symptomatic and supportive.
