What is Pantonix?

Pantonix is a proton pump inhibitor (PPI) containing Pantoprazole Sodium. It suppresses the final step of gastric acid production by covalently binding to the H+/K+-ATPase enzyme system on the surface of the gastric parietal cell, inhibiting both basal and stimulated acid secretion for over 24 hours.

Indications

Pantonix is indicated where suppression of acid secretion has therapeutic benefit: peptic ulcer diseases, gastroesophageal reflux disease (GERD), ulcers induced by NSAIDs, eradication of Helicobacter pylori (with antibiotics), and Zollinger-Ellison Syndrome.

Pharmacology

Pantoprazole is a proton pump inhibitor that suppresses the final step in gastric acid production by covalently binding to the H+/K+-ATPase enzyme system at the surface of the gastric parietal cell, leading to inhibition of both basal and stimulated gastric acid secretion.

Dosage & Administration

Benign gastric ulcer: 40 mg daily in the morning for 4 weeks, continued a further 4 weeks if not fully healed.
GERD: 20-40 mg daily in the morning for 4 weeks, continued a further 4 weeks if needed; maintenance dose 20 mg daily (may increase to 40 mg daily).
Duodenal ulcer: 40 mg daily in the morning for 2 weeks, continued a further 2 weeks if needed.
H. pylori-associated duodenal ulcer: 40 mg twice daily with Amoxicillin 1 g + Clarithromycin 500 mg both twice daily for one week, or Clarithromycin 250 mg + Metronidazole 400 mg both twice daily for one week.
NSAID-associated ulcer prophylaxis: 20 mg daily.
Zollinger-Ellison Syndrome: Initially 80 mg once daily, adjusted to response (elderly max 40 mg daily); doses above 80 mg given in 2 divided doses.
Tablet should not be split, chewed, or crushed.

Interaction

No clinically significant drug interactions have been observed in clinical studies with Pantoprazole.

Contraindications

Contraindicated in patients with known hypersensitivity to any component of the formulation.

Side Effects

Generally well tolerated. Headache and diarrhoea are common; abdominal pain, flatulence, rash, insomnia, and hyperglycaemia occur rarely.

Pregnancy & Lactation

US FDA Pregnancy Category B. No adequate well-controlled studies exist in pregnant women; use only if clearly needed. Pantoprazole is excreted in human milk — decide whether to discontinue nursing or the drug.

Precautions & Warnings

Long-term therapy may lead to malabsorption of Vitamin B12 or increase risk of osteoporosis-related fractures. Symptomatic response does not exclude gastric malignancy.

Overdose Effects

No known symptoms of overdosage in humans reported. As Pantoprazole is highly protein bound, it is not readily dialyzable. Treatment should be symptomatic and supportive.
