Diphenhydramine is a first-generation H1 antihistamine with anticholinergic, antiemetic, and sedative properties. It competitively blocks histamine H1 receptors, reducing symptoms of allergic reactions. Its CNS penetration provides sedative and anti-motion sickness effects.
Indications
- Allergic rhinitis and seasonal allergies
- Urticaria, pruritus, and insect bite reactions
- Motion sickness prevention and treatment
- Mild insomnia (short-term management)
- Nausea and vomiting including morning sickness
Dosage & Administration
Children 2–5 years: 5 mL (12.5 mg) every 4–6 hours. Children 6–12 years: 5–10 mL (12.5–25 mg) every 4–6 hours. Adults: 10–20 mL (25–50 mg) every 4–6 hours. Maximum 4 doses in 24 hours.
Contraindications & Precautions
- Hypersensitivity to diphenhydramine
- Neonates and premature infants

- Concurrent MAO inhibitor use
- Narrow-angle glaucoma, urinary retention, prostatic hypertrophy
Side Effects
Very common: sedation, somnolence. Common: dry mouth, blurred vision, urinary retention, constipation. Children may experience paradoxical CNS stimulation (excitability, insomnia).
Drug Interactions
CNS depressants (alcohol, sedatives, anxiolytics): additive sedation. MAO inhibitors: enhance anticholinergic effects. Tricyclic antidepressants: additive CNS/anticholinergic effects.
