What is Prasurel?
Prasurel (Prasugrel Hydrochloride) is an anti-platelet drug manufactured by Incepta Pharmaceuticals Ltd., used to reduce thrombotic cardiovascular events, including stent thrombosis, in patients with acute coronary syndrome managed with percutaneous coronary intervention (PCI).
Indications
Indicated to reduce thrombotic CV events in ACS patients managed with PCI: unstable angina (UA) or NSTEMI, and STEMI managed with primary or delayed PCI.
Pharmacology
Prasugrel is a thienopyridine prodrug that irreversibly inhibits the P2Y12 ADP receptor on platelets, preventing ADP-mediated activation of the GPIIb/IIIa complex. It has a mechanism of action similar to clopidogrel.
Dosage & Administration
Adults: single 60 mg loading dose, then 10 mg once daily, with daily aspirin (75-325 mg). Treatment of up to 12 months recommended.
Patients ≥75 years or <60 kg: after 60 mg loading dose, reduced maintenance dose of 5 mg once daily — the 10 mg maintenance dose is not recommended in these groups.
Interaction
Increased bleeding risk with warfarin and chronic NSAID use. May be co-administered with aspirin, heparin, GPIIb/IIIa inhibitors, statins, digoxin, and PPIs/H2 blockers.
Contraindications
Hypersensitivity to the active substance, active pathological bleeding, history of stroke or TIA, severe hepatic impairment (Child-Pugh class C).
Side Effects
Common: anaemia, haematoma, epistaxis, GI haemorrhage, rash, ecchymosis, haematuria, puncture site haemorrhage.
Uncommon: hypersensitivity/angioedema, eye haemorrhage, gingival bleeding.
Rare: thrombocytopaenia.
Pregnancy & Lactation
No adequate controlled studies in pregnant women. Not known if excreted in human milk; use during nursing only if benefit outweighs risk.
Precautions & Warnings
Increased bleeding risk in patients ≥75 years, <60 kg, with bleeding propensity, or on concomitant anticoagulants/NSAIDs. Discontinue at least 7 days prior to elective surgery.
