What is Veracal Injection?

Veracal Injection (Verapamil Hydrochloride) is an L-type calcium channel blocker manufactured by Incepta Pharmaceuticals Ltd., used intravenously for acute tachyarrhythmias and hypertensive crises.

Indications

Tachycardias including paroxysmal supraventricular tachycardia, atrial fibrillation with rapid ventricular response (except WPW), atrial flutter with rapid conduction, extrasystoles, acute hypertension, acute coronary insufficiency, and prophylaxis/therapy of ectopic arrhythmias during halothane anaesthesia.

Pharmacology

Verapamil is an L-type calcium channel blocker with antiarrhythmic, antianginal, and antihypertensive activity. As a negative inotropic agent, it should not be used in severe left ventricular dysfunction or hypertrophic cardiomyopathy.

Dosage & Administration

Adults: 5 mg slowly intravenously for tachycardias and hypertensive crises; may repeat after 5-10 minutes. Drip infusion: 5-10 mg/hour, up to 100 mg/day total.
Children: Newborn 0.75-1 mg; Infants 0.75-2 mg; 1-5 years 2-3 mg; 6-14 years 2.5-5 mg, given slowly IV under ECG control. Hypertensive crisis infusion: initially 0.05-0.1 mg/kg/hour, titrated up to 1.5 mg/kg/day.

Interaction

Increased plasma levels with CYP3A4 inhibitors (erythromycin, ritonavir), cimetidine; decreased with CYP3A4 inducers (rifampicin), phenobarbital. Increases plasma levels of digoxin, beta-blockers, statins, carbamazepine, and others. Potentiates hypotensive effect of diuretics/antihypertensives. Increased AV blocking effect with clonidine.

Contraindications

Severe left ventricular dysfunction, hypotension/cardiogenic shock, sick sinus syndrome, second/third-degree AV block, atrial flutter/fibrillation with accessory pathway (WPW), hypersensitivity to verapamil.

Side Effects

Constipation, nausea, hypotension, edema, CHF, pulmonary edema, bradycardia, AV block, dizziness, headache, fatigue, rash, elevated liver enzymes.

Pregnancy & Lactation

Use only if clearly needed; crosses placenta and detectable in umbilical vein blood. Excreted in breast milk — discontinue nursing during use.

Precautions & Warnings

Administer slowly under ECG control. Caution in first-degree AV block, bradycardia <50 bpm, hypotension, atrial fibrillation/flutter with pre-excitation, heart failure. May impair ability to drive; prolongs alcohol effects.
