What is Veracal SR?

Veracal SR (Verapamil Hydrochloride) is a sustained-release L-type calcium channel blocker manufactured by Incepta Pharmaceuticals Ltd., designed for once-daily dosing in hypertension and angina.

Indications

Essential hypertension, angina pectoris and prevention of re-infarction, supraventricular arrhythmias.

Pharmacology

Verapamil is an L-type calcium channel blocker with antiarrhythmic, antianginal, and antihypertensive activity. The sustained-release formulation allows once-daily dosing. It is a negative inotropic agent and should not be used in severe left ventricular dysfunction or hypertrophic cardiomyopathy.

Dosage & Administration

Dose individualized by titration; administer with food. Essential hypertension: 180 mg each morning initially. If inadequate response, titrate to 240 mg each morning, or 180 mg morning plus 180 mg evening, or 240 mg every 12 hours.

Interaction

Increased plasma levels with CYP3A4 inhibitors (erythromycin, ritonavir), cimetidine; decreased with CYP3A4 inducers (rifampicin), phenobarbital. Increases plasma levels of digoxin, beta-blockers, statins, carbamazepine, and others. Potentiates hypotensive effect of diuretics/antihypertensives.

Contraindications

Severe left ventricular dysfunction, hypotension/cardiogenic shock, sick sinus syndrome, second/third-degree AV block, atrial flutter/fibrillation with accessory pathway (WPW), hypersensitivity to verapamil.

Side Effects

Constipation, nausea, hypotension, edema, bradycardia, AV block, dizziness, headache, fatigue, rash, elevated liver enzymes.

Pregnancy & Lactation

Use only if clearly needed; crosses placenta. Excreted in breast milk — discontinue nursing during use.

Precautions & Warnings

Caution in first-degree AV block, bradycardia, hypotension, atrial fibrillation/flutter with pre-excitation, heart failure. May impair ability to drive; prolongs alcohol effects.
